Key Responsibilities

  • Delivers varied social work services to Home Health patients and their families by physician orders and under the direction and supervision of the Clinical Manager
  • Assists in the admission process of the patient to the Agency by performing an initial evaluation, assessing the patient's psychosocial status, and evaluating the patient, family, and home to identify socioeconomic and emotional factors that will affect the plan of treatment.

  • Assisting development and implementation of the interdisciplinary patent care plan as it pertains to medical social work.
  • Observes, records, and reports changes in the patient's emotional and social factors that affect the patient's illness and his/her need for care and his/her response to treatment.
  • Consults with the attending physician concerning the alteration of the planned treatment.
  • Maintains and submits written clinical records as deemed by the Agency, including the initial evaluation, the care plan, and daily notes.
  • Evaluates the patient's and family's response to, and effectiveness of, the medical social work intervention.
  • Confirms, every week, the scheduling of visits with the Director of Nursing to facilitate coordination of other staff visits.
  • Participates in Agency activities and committees when appropriate to include staff development activities and in-service education.
  • Supervises the social work assistant once monthly.
  • Participates in discharge planning.
  • Assumes responsibility for self-development by continually striving to improve his/her Medical Social Worker practice through formal education, attendance at workshops and conferences, active participation in professional and related organizational meetings, and individual research and reading.
  • Assesses the patient's potential to cope with his/her social health problems.
  • Acts as a consultant to the members of the health team, assist them in understanding the social, emotional, and environmental factors related to the patient's health problems.
  • Helps patients to utilize the resources of their families and the community.
  • Provides rehabilitative and supportive casework.
  • Assists patients and their families in coping with personal and environmental difficulties which might predispose them toward illness or interfere with obtaining maximum benefits from medical care.
  • The clinical record shall contain notes for each service provided. A clinical note will be completed for each visit made to a patient's home (including a private residence, assisted living facility, group homes, or skilled nursing facility) within 48 hours of the visit.

Professional Experience and Qualifications and Qualification


  • Ability to work independently, make accurate, and at times, quick judgments.
  • Ability to respond appropriately to crises outside of a hospital setting.
  • Acceptance of and adaptability to different social, racial, cultural, and religious modes.
  • Minimum 2 years of experience as a social worker, preferred. Active patient contact within the past three years is preferred.
Education

  • MSW Degree from a school of social work accredited by the Council on Social Work Education (CSWE), and one year of experience in a health care setting.
  • Baccalaureate Degree in social work (BSW) from a school of social work accredited by the CSWE, and one year of experience in a health care setting and be supervised by an MSW from a school of social work accredited by the CSWE and who has one year of experience in a health care setting. If the BSW is employed by The Care Team before December 2, 2008, he/she is exempted from the MSW supervision requirement.
  • Have a Baccalaureate Degree in psychology, sociology, or other field related to social work, and at least one year of social work experience in a health care setting and be supervised by an MSW from a school of social work accredited by the CSWE and who has one year of experience in a health care setting.
Regulatory Requirements

  • Licensed or registered in the State of Michigan.
  • Must pass a criminal background check & MVR check.
  • Completed health statements acknowledging the ability to perform the duties of the position.
  • Valid state driver's license.
  • Proof of current automobile insurance.
  • Current CPR card; On-line CPR certification accepted; Hands-on CPR certification preferred.
  • TB testing per agency policy; (1 or 2 step TB skin test within 12 months of hire & annual TB symptom screening thereafter).
