The Benefits Specialist will work with Santa Clara County homeless individuals to provide application assistance, eligibility determination, and enrollment assistance for Social Security Disability benefits, Covered California, and Medi-Cal.


  • Serve as appointed representative for Supplemental Security Income and Social Security Disability Insurance claims for homeless clients (previous experience representing disability claims is not required training will be provided).

  • Interface with medical teams and facilities to support disability claims and ensure access to primary care and mental/behavioral health services for homeless clients.

  • Provide health insurance outreach, education, and enrollment for Medi-Cal, Covered California, and county health coverage programs.

  • Engage directly with homeless clients in shelters to conduct needs assessments.

  • Complete training to become a Certified Enrollment Counselor and attend on-going state and federal health navigation trainings.

  • Track outreach data, maintain case files, and submit monthly progress reports of contractual goals.    

  • Assist clients with primary care, mental health, vision and dental referrals.

  • Attend shelter sites weekly case management meetings to update staff with clients current status with benefits and update sites of any new HCH program benefit information.

  • Assist clients with enrollment with GA/CalWORKs and Food stamp benefits.                           

  • Bachelor’s degree

  • Driver’s license

  • Thoughtful and creative thinker with an enthusiastic approach to work

  • Computer savvy and very strong writing skills

  • Excellent interpersonal skills and ability to interact effectively with diverse client population

  • Drive to seek solutions independently and contribute to team goals

  • Engaging public speaker
