We are looking for a Social Services Assistant who has experience in the healthcare setting, (preferrable skilled nursing). You will assist in maintaining policies and procedures to assure that the medically related emotional and social needs of the residents are met/maintained on an individual basis. You will assist the Social Services Director by interviewing residents and families to obtain history relevant to medical problems and planning. You will help resident and family to adjust to admission into the facility environment. Acts as liaison between medical and nursing staff, residents, relatives and appropriate outside agencies. You will assist in coordination of discharge planning for residents.

Successful candidates to be considered for the Social Services Assistant position will have the following:


  • Must possess, as a minimum, a high school diploma or its equivalent Associate or Bachelor's Degree preferred but not required.

  • Experience as a Social Services Assistant preferred

  • Experience in Long Term and Rehab care is a plus

  • Excellent communication skills is required

The primary purpose of your job position is to assist in planning, developing, organizing, implementing, evaluating, and directing our facility's social service programs in accordance with current existing federal, state, and local standards, as well as our established policies and procedures, to assure that the medically related emotional and social needs of the resident are met/maintained on an individual basis.
