A community hospital in Connecticut is actively seeking an Oncology Nursing Navigator to join their Cancer Center team.

 The Oncology Nursing Navigator will work at one of Connecticut’s comprehensive cancer care centers, known for their world-class surgeons, oncologists, and staff, along with their ongoing clinical trials and state-of-the-art technology and techniques. 


The Oncology Clinic Nurse Navigator will function as a consistent and primary contact for the patients and their families as they enter the often complex cancer care system. The Cancer Center Nurse Navigator will assist in coordination of care across the continuum for a designated patient population and provide guidance as patients transition from one therapy to another.


The Oncology Clinic Nurse Navigator will ensure that the patient and family have appropriate information, understand and have the capacity to adhere to the medical plan of care. The Cancer Clinic Nurse Navigator will be the bridge between various consultants, services and points of care experienced by the patient/family. 


The Outpatient Oncology Nurse Navigator will interface with point of care staff in accordance with the plan for patient care, educate patients and families based on determination of learning needs and communicate plan details to patient/family, physicians, advanced practice providers, intra-professional team members and others involved in patient care.


The Oncology Nursing Navigator must be a registered nurse holding their Bachelors degree, and ideally holding a clinical background working in either inpatient or outpatient oncology services. Although not required, Oncology Nursing Navigator will ideally hold (or be willing to obtain) their CN-BN or OCN certification. 


The Oncology Nursing Navigator will work at the Cancer Center located in southern Connecticut, in an area that is easily accessible from several cities and upscale suburbs of Connecticut and parts of New York. This area is home to many families, offering a dense suburban feel.
