Below is a list of job responsibilities for the RN Case Manager:

• Serve as a non-physician team leader.


• Assume responsibility for coordinating staff, programs, and services to maximize patient outcomes and quality care.


• Ensure patients are moving throughout the continuum of care.


• Minimize unnecessary resource utilization.
