The Opportunity

Homecare Homebase is searching for a Senior Clinical Optimization Consultant who will serve as a subject matter expert to consult with our clients to use of their technology, people and processes more effectively. This person will provide consulting services, recommendations and best practice guidelines to clients can improve their operations, grow their business and enhance clinical care. 

The Challenge

At Homecare Homebase, we help home health and hospice organizations across the nation overcome the business and technological challenges that stand in the way of patient care and efficiency. As Business Process Review & Clinical Optimization Consultant, you will play an essential role in evaluating how clients are using technology, and internal processes to uncover opportunities for improvement. Depending on the client's needs, your work may be focused on some or all of the following areas: Intake, Scheduling, Clinical Assessment / Care Delivery, Clinical Management and operations, Reporting / Analytics, Sales / Business Development, and Agency Management.

You will also be responsible for:


  • Objectively evaluating their current "As Is" state by surveying, interviewing, and/or conducting focus groups with constituents from across the organization

  • Reviewing current policies and processes against industry best practices

  • Helping clients define their desired "To Be" state

  • Assessing staffing and determining organizational realignment opportunities

  • Following the Optimization methodology and ensuring project team members are also following the methodology.

  • Guiding Clients towards established best practices.

  • Contributing and participating in the design, measure, analyze, improve and control phases of the optimization assessment/ implementation project

  • Investigating problems and needs, identify their root cause and determine possible solutions.

  • Assisting the Client to establish change control management and processes to allow for a successful maintenance post implementation of the Homecare Homebase application.

  • Providing holistic assessment and feedback on clinical performance to guide clinical and operational improvement. 

  • Working with Implementation Managers to implement strategies to improve team performance, foster team growth and assist with meeting department and company goals.

  • Serving as a SME on the proper use of the Homecare Homebase application as it pertains to clinical operations.

  • Assisting with Implementation work as needed, when Optimization schedule has availability

  • Assisting Clients to follow best practices, policies and procedures that maximize their use of the Homecare Homebase product.

  • Exemplifying CARES core values, promote a positive work environment and adherence to core values.

  • Following HIPAA policies within Homecare Homebase.

  • Travelling and/or consulting 30-70% in service of our clients across the country.

What We're Looking for

Homecare Homebase’s mission is to empower exceptional care among all the clients we serve.  The Senior Clinical Optimization Consultant supports this mission by providing careful observation and keen insight into the gaps that exist between an organization's people, processes, and technologies and its strategic goals.

We are looking for a talented, passionate individual who has at least 7 years' clinical experience and can show us:


  • A strong track record in Clinical operations at a home health or hospice organization

  • Critical thinking skills, analytical skills, problem solving skills

  • Experience with  ERP implementations and/or migrations involving Homecare Homebase

  • Talent for facilitating diverse groups and building a consensus as well as preparing an organization for change and improvement

  • Proven success conducting and implementing business process reviews in a homecare setting

  • The ability to map current and desired processes in detail, showing various process steps and who is performing each step

  • An understanding of homecare best practices and the ability to build out a portfolio of best practices for our clients

  • The ability to interact effectively with multiple constituent groups

  • The ability to function with minimal supervision without affecting the quality of their work.

  • A passion for Client service that ensures the needs of the Clients are met throughout their consulting engagement/ optimization implementation.

Education requirements: this career opportunity requires a bachelor's degree in business or a related field or an equivalent combination of some college and significant work experience. A Nursing Degree is desirable. Experience in clinical consulting, ability to analyze and solve problems and medical/healthcare experience is a plus.

As a Senior Clinical Optimization Consultant, you will be engaged as a full-time performance consultant with the majority of your work performed at the client site.
