The objective of the Access Center is to provide all outlying regional healthcare providers with easy access to the many physicians, specialties, and services at CHRISTUS Trinity Mother Frances Hospital. This position within the Transfer Center is responsible for the clinical review, triage, prioritization, and the coordination of the inter-hospital transfer to CTMFH. The Access Center RN in the Transfer Center will review the patient's clinical information to assist the sending physician and facility to obtain the appropriate physician or specialty needed for acceptance to CTMFH. The Access Center RN will demonstrate knowledge of disease processes, normal vs. abnormal clinical findings, and usual and customary healthcare treatments. From the clinical information, the RN will determine the level of care and a plan of care will be initiated. The Access Center RN will also review Interqual criteria to ensure that all admissions and transfers are appropriate and will assist in determination of the status of each admission.
This position will be directly responsible for the initiating of several critical clinical protocols, for example, Code STEMI and Code Stroke. The Access Center RN will coordinate all activities for these protocols including communication with physicians, appropriate interventional teams, and transportation, whether ground or air.
This position will work within day to day operations of Bed Control at CTMFH. This position will serve as an information resource and liaison for physician request for admission and/or transfers, nursing departments, clinical coordinators and other procedural or clinic areas requesting an inpatient, observation or bedded outpatient admission (this includes but is not limited to Surgery, ECC, PACU, Stress Lab, DDC, Physician Clinics and Registration, as well as, other hospitals requesting transfers in). This position will manage bed admissions, assignments of beds for patients and coordinate with nursing floors and housekeeping to maintain adequate & efficient patient throughput. This position will be the primary access point for physicians to direct admit to CTMFH. After obtaining the clinical information and admission order from the physician, the Access Center RN will assess the appropriateness of the admission and determine the appropriate patient status per MCG criteria.
