General description: Under the general direction of Collaborative Care Leadership, the Behavioral Health Coach (BH Coach) will be an integral part of the primary care team focused on providing support to patients with mild to moderate depression and/or anxiety. Works collaboratively with the team (including a SW and psychiatrist) to implement evidence-based services for primary care patients utilizing tools and training from the IMPACT Collaborative Care model and Mass General Brigham (MGB), engage/coach patients in self-directed web-based tools, such as iCBT (internet-based cognitive behavioral therapy), as well as connect patients to community providers when specialty treatment is indicated.
Principal Duties and Responsibilities:
Receive referrals from primary care and/or leverage EHR to proactively identify patients who would benefit from the program
Effectively communicate the intent of the team-based care program to the patient in order to enroll in intervention.
Conduct patient intake, including gathering information on past behavioral and mental health history and treatment, as well as information regarding pertinent psychosocial factors.
Demonstrate sound identification of safety concerns and escalate per workflow.
Leverage strong and efficient case presentation skills to review caseload with the psychiatric consultant and LICSW each week.
Ensure timely, systematic symptom monitoring of patients enrolled in the program using evidence-based screening tools (e.g., PHQ9, GAD7).
Use motivational interviewing to identify patient goals and use brief intervention techniques in order to engage patients in achieving their goals.
Leverage patient education and shared decision-making tools to assist with patient engagement and adherence to treatment plans.
Identify barriers to treatment adherence and work with patient on eliminating those barriers.
Maintain accurate and up-to-date records and standardized data on all patients.
Manage a target caseload of 60 adult primary care patients.
Collaborate with primary care team on patient goals and treatment.
Effectively engage primary care providers in referring to the program and collaborate on treatment plans
Participate in development of maintenance plan with patients to help them maintain a healthy lifestyle.
For patients not improving with IMPACT model, collaborate with Collaborative Care team to identify level of outpatient service best suited for patient, and assist patient in connecting with provider in community.
Attend mandatory trainings relating to work role as scheduled
Reporting Structure:
BH Coach will report directly to the Collaborative Care program manager for operational purposes, monitoring of workflow/protocols, and caseload review.
Qualifications
QUALIFICATIONS:
Master's of Social Work (MSW) or Mental Health Counseling or Psychology from an accredited program required.
Massachusetts Licensed Independent Clinical Social Worker (LICSW) or Licensed Mental Health Counselor (LMHC) or Licensed Clinical Social Worker (LCSW) with relevant clinical experience required.
May consider candidates with Bachelor's degree in social work, mental health counseling, psychology and 5 years' experience in relevant setting (health care/behavioral health).
Knowledge, Skills and Abilities:
Ability and willingness to function independently and proactively in a primary health care setting
Aptitude in problem-solving and behavioral change strategies (i.e. Motivational Interviewing and Behavioral Activation)
Ability to serve as a critical liaison to facilitate connections between people
Ability and willingness to work in a time-limited, structured, and solution-focused environment
Flexibility to adapt to unforeseen needs or circumstances
Ability to use electronic medical records once trained
Working Conditions:
Physician office
Ability to drive to MD offices

Location: PCP offices or a central location if space is not available
