BCForward is looking for Care Management Support Coordinator II Remote
Position Title:

Care Management Support Coordinator II

Location: Remote

Anticipated Start Date: ASAP

Duration: 03 Months Contract to Hire

Job Type: Full Time/ W2

Pay Range: $20/hr on W2

Education Level: High School

Shift (Start/End Time): 8:00am EST - 5:00pm EST

Overview of Responsibilities

  • Assist in activities related to medical and psychological aspects of utilization and coordinated care

  • Conduct outreach via telephone to assigned members daily

  • Maintain and update an Excel spreadsheet with member assignments

  • Communicate consistently with supervisor

  • Document all interactions in Virtual Health during phone calls

Performance Expectations/Metrics

  • Make a high volume of outbound calls daily

  • Utilize medical terminology appropriately

  • Maintain reliable home internet and dedicated workspace for HIPAA compliance (hardwired connection)

  • Complete 80-120 phone calls per day to the Medicare population

  • Achieve 100% compliance with assigned member tasks

  • Maintain an engagement rate of 80% or higher monthly

  • No time off permitted in the first 90 days except pre-planned during interview

  • Achieve a minimum audit score of 90%

  • Complete tasks for 600 members monthly (30 new members daily)

Required Skills/Experience

  • High school diploma or equivalent

  • Basic knowledge of Microsoft Office

  • Previous experience in a managed care setting, medical office, or facility with medical administration duties

Preferred Skills/Experience

  • Communication skills

  • Thorough knowledge of customer service, utilization review, or claims processing in managed care

  • Knowledge of medical terminology

  • Strong time management and organizational skills

Education Requirements

  • High school diploma or equivalent
