Overview
Supervise the activities and assume responsibility for all duties being performed by technicians, students, interns, and residents and monitors and evaluates their performance.
Interprets and evaluates prescriber's orders and for accuracy, completeness and appropriateness. Recognizes therapeutic problems, including drug interactions, contraindications, improper dosing, precautions of therapy, and potential / actual toxicities. Recognizes potential or actual problems and intervenes to recommend treatment to minimize the effects. Consults with practitioner when any questions or problems are identified.
Dispenses and maintains dispensing records for standard of care and investigational medications. Compounds unique dosage forms and strengths of medications used in investigational studies. Consults in the development and review of investigational drug protocols, as necessary.
Monitors therapeutic response and adverse effects of drug therapy. Proactively consults with practitioner when questions or problems are identified. Appropriately reports all adverse drug reactions, drug defects, and medication errors. Counsels and trains other health care professionals and patients regarding appropriate use of medications, (i.e., purpose, actions, dosage, side effects, storage, precautions, administration, etc.).
Responsibilities
BS in Pharmacy or Pharm.D.
Ability to meet the pharmaceutical care needs of acute care and ambulatory infusion/clinic patient care types
Pharmacist licensure in the State of Oregon
Job Related Knowledge, Skills and Abilities (Competencies):
Drug therapy in patients described in Position Purpose. (Dosage, indications, side effects, etc.) Contemporary medication use systems (interpret prescribers’ orders, prepare and dispense medications. etc.)
Pharmaceutical care principles
Age-related modification of drug therapy
Disease-related modification of drug therapy
Contemporary medication use systems
Interpersonal, formal and informal communication, including presentations
Project management concepts
Systematic problem solving skills,
Ability to work independantly with minimal supervision
Excellent interpersonal communication skills
Qualifications
Advanced Pharmacy Practice Residency
Experience in Oncology pharmacy practice, either inpatient or outpatient.
Experience in Investigational Drug Service/ Research Pharmacy practice.
Experience in sterile compounding processes USP 797 and hazardous drug handling USP800.
