HCAM offers diverse connection and coordination services for individuals and families across Maryland through various programs. HCAM’s Care Coordination Program (CCP) provides short-term care coordination services to Baltimore City residents who are eligible for Medicaid benefits and need help accessing services. The Care Coordination Program strives to improve birth outcomes and protect our city’s most vulnerable populations in Baltimore City though outreach strategies and providing access to health services and community resources by:
The RN Care Coordinator will serve as a part of CCP’s Administrative Care Coordination Unit (ACCU) and will provide short term care coordination services to individuals and families.

While performing the duties of this job, the employee is regularly required to talk, hear and respond to customers and employees. The employee frequently is required to stand; use hands to finger, handle, or feel; reach with hands and arms; climb or balance; and stoop, kneel, crouch, or crawl. The employee is occasionally required to walk 3 blocks or more and sit for prolonged periods of time. The employee must regularly lift and/or move up to 10 pounds and occasionally lift and/or move up to 15 pounds. Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception, and ability to adjust focus.

WORK ENVIRONMENT
While performing the duties of this job, the employee is exposed to both indoor and outdoor conditions. The employee will be subject to noise.
