Responsibilities:

• Provide direct nursing care to residents under the supervision of a Registered Nurse (RN)


• Administer medications and treatments as prescribed


• Monitor residents' condition and report any changes to the nursing supervisor


• Assist with activities of daily living, including bathing, grooming, and mobility


• Collaborate with interdisciplinary team members to develop and implement individualized care plans


• Maintain accurate and up-to-date medical records and documentation

Qualifications:

• Current LVN license in the state of (insert state)


• CPR certification


• Prior experience in long-term care or skilled nursing preferred but not required


• Excellent communication and interpersonal skills


• Ability to work effectively as part of a team
