*PT/PRN hours are based on working 59 hours or less per two-week pay period


SCOPE OF ROLE


Reporting to the Manager, Ryan White Program, the Dietician will play a crucial role in our outpatient primary care services and our Ryan White program, providing nutritional guidance to a diverse patient population, including those living with HIV. As part of an integrated healthcare team, the Dietician will be instrumental in conducting thorough assessments, designing personalized nutritional plans, and contributing to the overall wellness of our patients.


HOW YOU’LL SUCCEED:



  • Conduct nutritional screenings, assessments, and follow-up monitoring for patients through both in-person and telehealth supported visits.

  • Develop and implement individualized nutritional care plans for patients, including those with HIV, chronic illnesses, and other health conditions.

  • Provide nutritional education and counseling to patients and their families, leveraging telehealth platforms to enhance accessibility and convenience.

  • Monitor and document patient progress and adjust nutritional plans as necessary, maintaining high standards of care in both virtual and face-to-face interactions.

  • Collaborate with an interdisciplinary team including physicians, nurse practitioners, mental health professionals, and other healthcare staff to deliver comprehensive care.

  • Support dietary services in Signature Health's residential treatment facilities as needed, including nutritional assessments, meal planning, and supervision of therapeutic diets. 

  • Participate in case conferences and team meetings to discuss patient care and progress, utilizing telehealth tools as necessary.

  • Support the goals and objectives of the Ryan White program within the clinic.

  • Adhere to all policies and procedures established by Signature Health, ensuring compliance with organizational standards.

  • Uphold patient confidentiality and privacy in accordance with HIPAA regulations, promoting a secure and respectful environment.

  • Complete all mandatory compliance training as provided by Signature Health, demonstrating commitment to professional development and ethical practices.

  • Maintain consistent attendance and punctuality.

  • Perform additional duties as assigned, flexibly adapting to evolving organizational needs. 


KNOWLEDGE & EXPERIENCE:



  • Registered Dietitian (RD) by Commission of Dietetic Registration required.

  • Valid unencumbered state licensure if required by state law.

  • 2+ years’ of clinical dietician experience required.

  • American Heart Association (AHA) Basic Life Support (BLS) certification required.

  • Basic PC software application knowledge and ability to use multiple data systems preferred.


WORKING CONDITIONS:



  • Work is normally performed in a typical interior/office/clinical work environment.

  • While hours of operation are generally standard, flexibility to work evenings and extended hours may be required.

  • Requires periods of sitting, standing, telephone, and computer work. 

  • Hearing: adequate to hear clients or patients in person, over the telephone or through telehealth technology.

  • Speaking: adequate to speak to clients or patients in person, over the telephone or through telehealth technology.

  • Vision: Visual acuity adequate to perform job duties, including reading information from printed sources and computer screens.

  • Physical effort required: occasional lifting and carrying items weighing up to 15 pounds, unassisted.

  • Possible exposure to blood borne pathogens while performing job duties.

  • Frequent bending, reaching, and repetitive hand movements, standing, walking, squatting and sitting, with some lifting, pushing and pulling exerted regularly throughout a regular workday.

  • Sufficient dexterity to operate a PC and other office equipment.
