Judge Healthcare is seeking a Nurse Practitioner for an opening with our biggest client!
Scope of Role:
The Nurse Practitioner (NP) within the Senior Community Care (SCC) program plays a crucial role in coordinating and delivering clinical care to our highest-risk health plan members. SCC is a longitudinal, integrated care delivery program that operates in a facility setting. The NP collaborates with an interdisciplinary team to provide comprehensive care.
Responsibilities:
Conduct Comprehensive Assessments:
Perform thorough assessments of patients' health status, including physical, mental, and social aspects.
Identify health risks and develop appropriate care plans.
Manage Acute and Chronic Conditions:
Effectively manage acute and chronic medical and behavioral conditions.
Prescribe medications, order diagnostic tests, and interpret results.
Accurate Documentation:
Ensure accurate and complete documentation, including ICD-10 coding.
Maintain electronic health records (EHRs) with attention to detail.
Collaborate with the Interdisciplinary Team:
Communicate and collaborate with other healthcare professionals, including physicians, nurses, social workers, and therapists.
Participate in care conferences and case reviews.
Advanced Illness and Care Planning:
Engage in advanced illness conversations with patients and their families.
Assist with advanced care planning, including end-of-life discussions.
Patient Education and Counseling:
Provide patients and caregivers with education on managing health conditions.
Offer counseling related to lifestyle modifications, preventive measures, and treatment options.
Required Qualifications:
Active and unrestricted license as a Nurse Practitioner in the state where you reside or the ability to obtain one by the start date.
Graduation from an accredited Master of Science in Nursing (MSN) or Doctor of Nursing Practice (DNP) program.
Board certification through the American Academy of Nurse Practitioners (AANP) or the American Nurses Credentialing Center (ANCC) in one of the following specialties:
Family Nurse Practitioner
Adult Nurse Practitioner
Gerontology Nurse Practitioner
Adult-Gerontology Acute Care Nurse Practitioner
Current, active DEA licensure/prescriptive authority or the ability to obtain it post-hire, per state regulations.
