Job Description:

GENERAL SUMMARY/ OVERVIEW STATEMENT:



Direct Patient Care



PRINCIPAL DUTIES AND RESPONSIBILITIES:





  • Direct ongoing primary care of patients


  • Provision of full range of ambulatory services to both well and sick adult, adolescent, child, elderly, disabled, or cognitively impaired patients including routine health care maintenance as well as episodic care for illness/injury, etc.


  • Provide chronic disease management including outpatient addiction services including medication assisted treatment and chronic pain


  • Participate in integrated behavioral health treatment interventions as needed


  • Respond to all medical emergencies in the Health Center as appropriate


  • Participate in ongoing quality assurance programs


  • Follow patients admitted to Mass General Hospital as needed with care coordination after discharge planning


  • Attend Adult Medicine Department and Pediatric Department staff meetings


  • Participate in scheduled night and weekend coverage of Health Center services


  • Participate in population management and PCMH activities


  • Participate in projects and initiatives as needed for CMO or CEO







Qualifications

QUALIFICATIONS:




  • Education: Graduate of an accredited medical school and residency program in Family Medicine


  • Licenses: Current licensure in MA and current DEA and DPH Controlled Substance Certificates mandatory





SKILLS/ ABILITIES/ COMPETENCIES REQUIRED:





  • Abide by the By-Laws, rules, regulations and policies of the North End Waterfront Health


  • Limit practice to those privileges delineated & approved by the CEO & CMO in connection w/application for appointment and reappointment


  • Accept consultations, committee and other assignments from the health center as requested


  • Attend MD staff meetings as required by Department


  • Adhere to the profession's clinical standards and Code of Ethics


  • Promote patient satisfaction


  • Adhere to the North End Waterfront Health's standards, policies and procedures


  • Promote North End Waterfront Health's public relations


  • Be responsible for own professional development


  • Be willing to maintain Massachusetts licensure at highest level of eligibility
