Job Description:

Primary Functions:

This position is responsible for coordinating all aspects of Hematopoietic Progenitor Cell (HPC) from blood and marrow transplantation of pediatric recipients. The Transplant Coordinator provides follow up before, during and after transplant. The coordinator functions as a liaison between local and referral institutions and collaborates with members of the health care team to enhance patient care during the transplant process. The Transplant Coordinator also acts as a clinician, educator, consultant and leader to enhance patient care by developing, supporting and educating nursing staff and others, She/he makes clinical judgements based upon a synthesis of current research, concepts, principles, theories and outcomes to solve complex problems during overall transplant process. The Coordinator is also responsible for the data management operations.

Essential Functions:

1. Works directly with Transplant Medical Director to assure appropriate scheduling of referred patients for transplantation

2. Takes initial intake information on all new patient referral,and assures that insurance approval for transplantation is obtained in a timely manner. Also works with referring institutions to educate and updated staff transplant process.

3. Meets with all potential transplant patients and their families to establish contact and initiate education on

Transplantation process.

4. Coordinate, schedule and assist in all stem cell collections and bone marrow harvests. Monitor patient's test during mobilization of stem cells.

5. Assures that all pre-transplant evaluation is completed prior to transplant admission.

6. Serves as a liaison to update transplant medical director, outpatient nurses, data manager and referral centers.

7. Assist with discharge planning of all patients and clinic follow-ups. Assures that appropriate discharge information

is communicated to the clinic staff and to referral centers once patient returns back home.

8. Maintains continuity of care for all transplant patients and serves as primary contact for patients and families.

9. Coordinate donor search through cord blood bank and National Marrow Donor Program registries and work closely

with transplant medical director in selecting suitable donors for transplantation.

10. Assures that patients are assigned to all open and appropriate studies and that consents are obtained.

11. Completes BMT data reporting of information in a timely manner to all necessary organizations as advised by the Program Director and in accordance with federal legislation

Job Requirements:

Registered Nurse with Bachelor's Degree

Oncology experience preferred

1-3 years of job related experience required

BLS from American Heart Association
