Job Description:

DHHS is seeking a Health Equity Coordinator to coordinate and implement statewide and local Cardiovascular Health Learning Collaboratives to address cardiovascular health in Nebraska communities.


This role will also lead the development of and implementation of projects with healthcare clinics/organizations and community organizations focused on the identification and management of hypertension.


Duties include:
* Organizing and facilitating the statewide and local learning collaborative, planning activities.


* Building and managing a coordinated network of multi-disciplinary partners, collecting and tracking information on action plans and outcomes, completing necessary reporting, and creating and communicating meeting agendas, meeting minutes, and action items.


* Developing subject matter expertise and presenting information to a range of stakeholders on the Heart Disease and Stroke health initiatives is expected.


* Assist in providing technical assistance and monitoring to contractors/subrecipients, healthcare clinics/organizations, local health departments and other partners on evidence-based strategies, policies, and system changes to improve hypertension, high blood cholesterol, heart disease, and stroke prevention and management.


* Support initiatives and activities of the Heart Disease and Stroke Program, including the contribution to the development of content for work plans, budgets and progress reports and participating on committees and work groups to promote and enhance programmatic development and meeting program-established deliverables.


Schedule: 8-hour days Mon-Fri ; Some Flexibility between 7:30am-5pm



Requirements / Qualifications:




Minimum Qualifications: Bachelor's degree or equivalent coursework/training in community health education, biological science, adult/continuing education or related field AND experience in public/community health education activities with responsibility for: developing, evaluating, and leading health education program activities; assessing health needs; writing and designing educational and informational material, training aids, and presentations; preparing grant proposals; or planning and leading group sessions. Related experience may substitute for a Bachelor's degree on a year-for-year basis.



Preferred Qualifications: Master's degree in public or community health, healthcare, or related field. Minimum of one year of experience working in public or community health, healthcare, or related field. Experience in or knowledge of: health care system interventions to improve the delivery and quality of care in clinical settings that lead to early detection, management, and prevention of heart disease and stroke; and connecting community programs with health systems to improve heart disease and stroke prevention, care, and management.



Other: Valid Driver's License



Knowledge, Skills, Abilities:




Knowledge of the principles and practices of public health planning and implementation; the principles of grant writing, reporting, and grant administration; the practices and techniques of developing partnerships and collaborations.



Ability to maintain accurate and thorough records, design educational and informational materials, training aids, and presentations; prepare grant proposals; plan and lead group sessions; interpret and apply federal grant requirements, agency rules, policies, and procedures; communicate effectively; prioritize and manage time; problem solve and make decisions; demonstrate professionalism; balance needs, requirements, and workload for multiple projects; self-motivate and work independently with minimal supervision.
