Job Description:

The School Based Care Coordinator is responsible for assisting individuals with complex chronic Mental Health, Substance Use, and/or Intellectual Developmental Disability diagnoses navigate the behavioral health, physical health, and school system. The coordinator shall provide a person-centered approach to identify service needs, link to the most effective level of care, use a system of care approach in collaboration with stakeholders, monitor quality of care/ use of evidence-based best practice, and ensure consistent collaborative approach with school system.

This position will require travel within Wake County and to the home office (Morrisville, NC) as needed for meetings. We anticipate a projected start date of July 8,2024

Responsibilities and Duties

Planning and Care Coordination


  • Assure that clinical assessments of individuals in identified Special Needs Populations are completed in order to identify needs for treatment or monitoring

  • Identify gaps in services and intervene to assure that the individual receives appropriate care

  • Assure that services for the individual are coordinated across the MCO and with other systems, including primary care and identified school system.

  • Measure results of intervention and treatment, including reduction in high-risk events and inappropriate service utilization

  • Provide clinical discharge planning assistance to individuals discharged from state/local hospitals, PRTF’s, and other residential settings to assure that they follow up with aftercare services, receive needed assistance to prevent further hospitalizations, and support successful transition back to school setting

  • Proactively assure that members have a behavioral health and a medical home

  • Collaborate with school district to address school related concerns, school safety plans, behavioral support and academic transition plans, and special education needs

Communication and Collaboration


  • Works in partnership with other departments to address identified needs of all members

  • Proactively respond to an individual’s planned movement outside the MCO geographic area to ensure continuity of care, assist with changes between insurance plans, and address barriers to services due to guardianship

  • Family engagement to support child/family through crisis/treatment episode, educate on role of School Based Team, obtain release of information, link to appropriate supports/services, and connect with WCPSS

  • Participation in Child and Family Team meetings and/or safety planning meetings

Documentation and Record Review


  • Assure that all clinical documentation (e.g. goals, plans, progress notes, etc.) meet state, agency, team, and Medicaid requirements

  • Maintain medical record compliance/quality, as demonstrated by compliance on QM record reviews, peer reviews, referral source surveys, and individual/group supervision

  • Proactively monitor own documentation to ensure that issues/errors are resolved as quickly as possible

Knowledge, Skills, and Abilities


  • Ability to be diplomatic and use discretion to effectively negotiate and resolve issues with minimal assistance.  

  • Ability to communicate and interact effectively with others, and the propensity to make prompt independent decisions based upon relevant facts.  

  • Skill in problem solving, negotiation, arbitration and conflict resolution to balance the needs of both internal and external stakeholders. 

  • Skilled at shifting between macro and micro level planning, maintaining both the big picture and seeing that the details are covered.  

  • Knowledge of the Diagnostic and Statistical Manual of Mental Disorders (current version). 

  • Knowledge of the MH/SUD/DD service array including residential and crisis services. 

  • Knowledge in Medicaid B and C waivers and URAC accreditation.  

  • Ability to organize multiple tasks and priorities, collaborate within multiple systems, and effectively manage projects from start to finish.

  • Ability to shift work activities quickly according to mandated changes and changing priorities within the department.  

  • Knowledge of Microsoft Office products (such as Word, Excel, Outlook, PowerPoint, etc.).  

Minimum Requirements

A Master’s degree in a Human Services field (such as Psychology, Counseling, Social Work, etc.) is required, and two (2) years of post-degree progressive experience providing similar services to the population served.  Additionally, valid licensure or provisional license in profession (a LCSW, LCMHC, LMFT, or LCAS) is required.

Employee is responsible for complying with respective licensure board’s continuing education/training requirements in order to maintain an active license.    

Additional Training Preferred:

Experience in the public mental health field and school system is highly desired due to the complex work of the program.

Salary Range

$58,480 - $76,024/Annually

Exact compensation will be determined based on the candidate's education, experience, external market data and consideration of internal equity.
