The University of Iowa Hospitals and Clinics Department of Nursing is seeking a Discharge Nurse Clinician to perform specific tasks to assist members of the Nursing team in providing patient education and discharge planning under the supervision of the Director of Pediatric Care Coordination Division.
Responsibilities will include, but are not limited to:
Coordinate and/or assist in the planning process for discharge of patients
Serve as a resource and liaison for bedside nurses, patients and families for children being discharged from SFCH
Provide follow-up phone calls to families of children post discharge from the hospital
Assist with discharge planning including providing education to patients and families to provide a smooth transition from hospital to home
Regular salaried position located in Iowa City, IA

Required Qualifications:
A Baccalaureate degree in Nursing
Reasonable (1-3 years) recent pediatric nursing experience in children's hospital or academic healthcare environment
Current license to practice nursing in Iowa
Demonstrates expertise with discharge planning for pediatric patients with complex health care needs (involving multiple departments and external agencies).
Demonstrated expertise in specific clinical area of practice
Proficiency in computer applications including Microsoft Excel, Word and PowerPoint
Excellent written and verbal communication skills
Demonstrated excellent organizational skills, critical thinking, and problem-solving skills
Experience working with an interdisciplinary team
Experience in quality management
Desired Qualifications:
Previous case management experience is desired
Certification in case management/pediatric nursing is desired
Pervious experience with EPIC is desired
