  • Coordinates in tandem with the Day Program Coordinator all activities related to the PCP process to ensure compliance with COMAR regulations, Long Term Services and Supports (LTSS) requirements, and internal procedures.
  • Reviews the PCP for quality and accuracy to ensure the PCP reflects the person's strengths, needs and supports. Actively works with the person and the team to develop person-centered outcomes and goals for the person to achieve his/her "good life".
  • Attends PCP meetings including preparation meetings, annual meetings, and interim meetings.
  • Works with leadership to identify funding needs for potential admissions and for current persons served who may require additional short or long-term supports.
  • Tracks the PCP process to ensure implementation and documentation adhere to regulations.
  • Maintains a list of services provided to all persons supported and notes changes in plans including the date of the change and service units throughout the year. Notifies the program staff and business office staff as changes occur.
  • Monitors LTSS website daily for pending plan activity and acts accordingly.
  • Supports the Division Manager as needed with the review of LTSS and acceptance of services. Compares requested service units as outlined in each person's plan as submitted to data entered into LTSS and confirms accuracy. Follows up with the Division Manager and/or the Coordinator of Community Services to discuss discrepancies before accepting the services.
  • Audits ISP data for weekly billing and communicates with the appropriate personnel.
  • Collaborates with applicable team members, including the nurse, day program personnel, Day Program Coordinator, House Managers, Division Managers, Director of Person-Centered Planning, etc. as it relates to the PCP process, implementation, data collection, and information updates.
  • Review and update the person's record annually and as changes occur throughout the year through Chimes' electronic record keeping.
  • Ensures the Individual Support Plan (ISP) programs are updated annually before the annual plan date and as changes occur utilizing Chimes' electronic record-keeping
  • Facilitates the process for completion of Restriction Authorization forms to be presented to the standing committee for approval.
  • Trains the Division Manager/Supervisor/Day Program Coordinator relative to the PCP implementation and ISP data entry. Will support training of Direct Support Professionals as needed.
  • Regularly monitors LTSS to ensure that discharges from Day and Employment Services are reflected in LTSS as they occur. Follows up with LTSS staff when information is not maintained timely.
  • Completes the PCP's for potential admissions.
  • Participates in Behavior Plan (BP) meetings and reviews of BP's.
  • Submits reports to the Division Manager, Director of Person-Centered Planning or designated entities.
  • Participates as needed in standing committee meetings and other meetings and workgroups as assigned.
  • Uses all available technology for the completion of specified job duties.
  • Demonstrates knowledge of and compliance with state and agency regulations and requirements and can articulate these to stakeholders.
  • Contributes to a work atmosphere in a manner that is positive, enthusiastic, respectful, and courteous.
  • Attends work regularly according to the assigned work schedule and by Agency policy. Attends and participates in in-service training, staff meetings, and other activities to facilitate professional development.
  • Is a positive role model for persons served and Agency staff.
  • Works cooperatively with others including all staff, supervisors, administrators, co-workers, persons supported, community professionals, customers, vendors, and the public. Works independently, follows instructions, and abides by Agency policies and procedures.
  • Assumes other duties, responsibilities, and special projects as needed.

  • REQUIREMENTS:EDUCATION: Bachelor of Arts or Sciences degree from an accredited college or university with emphasis in the areas of education, psychology, social science, or a related field. Master's degree in a related field preferred. Valid CPR and FA preferred.EXPERIENCE: Minimum of 2-4 years of experience in program development and implementation with a minimum of 2 years of managerial or supervisory experience for the same. Experience providing community services to the developmentally delayed or IDD community is preferred. Must have an acceptable driving record as determined by criteria established by the Agency's insurance carrier and by the Agency policy. Must be proficient in data entry and detail-oriented. NOTE: At the discretion of the Personnel Officer, additional related experience and/or education may be substituted in lieu of the requirements specified above under Education and Experience.
