We are seeking a dedicated and experienced Social Worker Supervisor to join our team and play a vital role in ensuring the well-being of our residents. In this position, you will be instrumental in leading and managing the Social Services Department, while also providing direct social work services to residents and their families.


What You'll Do:


Lead the Way: You will actively participate in planning, developing, implementing, evaluating, and overseeing our entire social services program.


Resident Advocate: Conduct thorough resident assessments, participate in discharge planning, and develop individualized social care plans that address each resident's unique needs.


Connecting with Care: Build strong relationships with residents and their families through interviews, providing social/emotional support, and connecting them with valuable community resources.


Empowering Others: Offer expert consultation to staff and community agencies on best practices for meeting residents' social and emotional needs.


Championing Confidentiality: Ensure the highest level of privacy and security for all resident information.


Administrative Excellence: Contribute to developing and maintaining department policies and procedures, complete documentation, and assist in tasks such as arranging transportation and financial aid assistance.


You're a Perfect Fit If You Have:


A Master's degree in Social Work (or a Bachelor's degree with extensive experience) and at least 1 year of supervised experience in a healthcare setting.


A valid Massachusetts Social Work License.


Strong leadership skills with the ability to delegate tasks effectively.


Excellent communication and interpersonal skills to build rapport with residents, families, and staff.
