Job Summary | This position is responsible for assessing individual self-neglect in adults as part of the College of Social Work Adaptive Response Initiative, in partnership with the Department for Community Based Services (DCBS) Adult Protection Services Branch (APS). This position will serve as part of the DCBS APS team to provide oversight and evaluation of multidimensional, multidisciplinary assessments of functional ability, physical/cognitive/mental health, and socioenvironmental factors to determine required level and frequency of services. This position is responsible for the management and evaluation of individual plans of care, in collaborationwith community partners, designed to promote independence, autonomy, and dignity. |
