Description: Judge Healthcare is actively seeking a part-time Nurse Practitioner for Home Assessments with a Healthcare system based in Franklin or St. Charles Counties, MO!
Terms: Part-time, Flexible weekly schedule!
This job will have the following responsibilities:
Conduct in-home assessments on health plan members
Check vitals and conduct physical exams
Identify needs and opportunities to generate clinical referrals
Identify diagnoses to be used in care management
Educate members on various topics
Qualifications & Requirements:
NP license and national certification (AANP or ANCC) required
Minimum of 1 year clinical experience required, home health care or home visit experience is a plus
Family, Geriatrics, or Adult certification required
Excellent administrative, organizational and verbal skills
Reliable transportation in order to complete home visit assessments
