Summary:



Evaluates and treats patient populations for physical therapy needs.



Responsibilities:



Performs and documents initial and ongoing assessments of patient's condition.Educates and counsels patients and families regarding treatment plans and progress. Provides information regarding appropriate selection and use of adaptive equipment and support resources.Plans or assists with patient discharge from therapy services.Designs and implements a plan of care which includes frequency and duration of planned treatment and is appropriate to issues identified.Supervises activities of physical therapy assistants, students and other support personnel.
