Our client is currently seeking a Case Manager RN to work in Everett, WA.

Type of Engagement: Contract



This job will have the following responsibilities:

• Hospital Transition calls for Medicare Advantage patients


• Care Coordination for complex, high-risk patients


• Motivational interviewing with patients and families


• Palliative Care conversations when appropriate


• Participation in Interdisciplinary


• Team meetings

• Ambulatory Care


• Outpatient Care Coordination


• Difficult conversations


• Care Management
