Eskenazi Health serves as the public hospital division of the Health & Hospital Corporation of Marion County. Physicians provide a comprehensive range of primary and specialty care services at the 333-bed hospital and outpatient facilities both on and off of the Eskenazi Health downtown campus including at a network of Eskenazi Health Center sites located throughout Indianapolis.

Job Role Summary
The Community Outreach Worker promotes education on topics with emphasis on public health concerns which most affect the target audience. Shares general information on health and other issues through presentations in group formats and interacts on an individual basis, as needed. Performs basic health screenings, completes referrals for individuals who require additional assistance and works with other organizations to bridge the gap between available resources and the client needs. Provides support to the Community Outreach Coordinator with daily tasks, as required, in order to meet assigned goals and objectives.
Essential Functions and Responsibilities
Proactively contributes to Eskenazi Health's mission: Advocate, Care, Teach and Serve with special emphasis on the vulnerable population of Marion County
Provides preventive health education to patients and their families
Informs clients of health and community resources that are available to them in English and Spanish
Completes daily report for the Mexican Consulate
Manages data and updates spreadsheet on a weekly basis
Assists in preparation of the monthly and quarterly reports
Conducts follow-up with clients on status of referrals and other services
Facilitates VDS workflow on and off-site
Supports the Community Outreach Coordinator to help ensure timely and effective delivery of services
Attends meetings as directed
Job Requirements

Bilingual in English and Spanish and bicultural in the Hispanic/American culture
Communicates clearly with patients, families, Mexican Consulate and Eskenazi Health personnel
Treats all patients with dignity and respect
Possesses a good understanding of Microsoft Word and Excel
Ability to operate a blood pressure machine
Ability to operate a blood glucose meter and read test results
Attends health fairs at parks, churches, and other community locations
Knowledge, Skills & Abilities
Associates degree preferred
A minimum of two-years of experience working with community organizations is preferred
Valid Indiana driver's license required as well as proof of insurance
