Job Description:

Wyoming Department of Family Services requires that every child/adolescent in the care and custody of the Department of Family Services (DFS) will have an independent assessment prior to admission to a Qualified Residential Treatment Program (QRTP) per the Families First Prevention Services Act. The Qualified Individual Assessor will conduct Initial Qualified Individual Assessments (QIAs) and provide level of care recommendations to DFS upon review of the youth's medical records, interview of collateral contacts, interview of youth and guardian, and completion of a Child and Adolescent Needs and Strengths assessment. The QI will also assist with administrative duties required for oversight and management of contract requirements, to include data entry, assessment submission, monitoring of timelines, and participation in team meetings as needed.


  • Determine if the needs of youth referred for assessment by DFS can be met with family members or through placement in a less restrictive setting. Level of care recommendations must ensure the least restrictive placement environment for the child and be consistent with short and long-term goals for the child, as specified in the permanency plan for the child.

  • Document short and long-term mental health and behavioral goals for each youth within the Qualified Individual Assessment.

  • Conduct interview of youth and guardian at a time and location convenient for them.

  • Maintain timely submission of all assessments in compliance with contract requirements. Proactively notify leadership of any potential barriers to timely submission of assessments to allow for request of administrative extension of timeline as needed.

  • Complete administrative tasks to include data entry of new referrals in internal database and submission of completed Initial assessments as a back-up to other team members, as needed.
