The triage nurse is a licensed registered nurse who participates in the plan of care to provide competent, compassionate and individualized nursing care to patients under the direction of a physician or advanced practice provider. The RN also facilitates safe and efficient practice operations and acts as a liaison to provide and ensure continuity of care for the patient and family.


Principal duties and responsibilities include:


  • Triage all clinical patient calls and messages, collecting all pertinent information regarding symptoms and acuity, to enable physicians to develop a timeline for plan of care

  • Schedule urgent sick visits as appropriate.

  • Provide clear, accurate, and timely documentation of all patient contact.

  • Collect specimens in clinic and instructs patient about specimen collection at home, as applicable

  • Performs other tests, procedures, and treatments within the RN scope of practice.

  • Administer medications/vaccines within scope of nursing practice

  • Perform oximetry and vital signs when appropriate.

  • Act as first responder for medical emergencies in clinic

  • Refill prescriptions for medications within scope of nursing practice

  • Assist DME coordinator as needed with prior authorizations for all durable medical equipment and various diagnostic test orders, including set up of home oxygen, nebulizers, CPAP, etc. to patients.

  • Provide guidance and assistance when needed to medical assistants completing prior authorizations for pulmonary medications.

  • Perform designated tasks to track test results and communicate results to patients for physician, if appropriate.

  • Work with the clinical director and practice manager to define standards of excellence for patient care

  • Participate in and contributes to performance improvement activities

  • May be asked to cover other sub-specialty RN positions in the department






Qualifications


  • Knowledge, skills and ability to assess patient/family needs

  • Ability to maintain effective working relationships with patients/families and staff

  • Excellent interpersonal, written and verbal communication skills

  • Current licensure as a registered nurse in the Commonwealth of Massachusetts

  • Bachelor's degree in Nursing required

  • 2 or more years of nursing experience



Working conditions in an outpatient office setting with a shared office. Hybrid schedule optional, schedule is four 10-hr shifts.
