Sandra Eskenazi Mental Health Center, Indiana's first community mental health center, provides comprehensive care for emotional and behavioral problems, including severe mental illness and substance abuse. The Sandra Eskenazi Mental Health Centeroffers both inpatient and outpatient services, including several outreach centers as well as clinic- and community-based services.




About the Prevention and Recovery Center (PARC)




PARC CSC provides early screening, prevention and intervention services for individuals ages 14 to 35 who have experienced a first episode of psychosis within the past five years.


PARC CSC provides comprehensive clinical services with multidisciplinary teams evaluating and addressing every participant's individual needs. Each team member completes training specific to first episode psychosis and adheres to the evidence-based OnTrackNY model of care.


Three clinical teams serve program participants, with each comprised of a psychiatrist, psychologist, therapist, case manager, nurse, clinical pharmacist, supportive employment and education specialist, and a team lead. Additional staff provide outreach and educational services in the community and across the state of Indiana. The PARC CSC program aims to improve functioning, promote wellness and recovery, and ultimately improve outcomes for young people with psychotic disorders.

Where You Fit In:




The PARC CSC Care Coordinator collaborates with the MD/CNS and the treatment team, plays a vital role in formulating, developing, and revising treatment plans, conducting assessments such as BPSA's, ANSA, CANS, and other client assessments. The Care Coordinator serves as a dedicated advocate for clients, ensuring their access to essential services by cultivating community relationships and resources.

Am I Qualified?






  • Bachelor's degree required, with the ability to meet minimum competency standards established by Sandra Eskenazi Mental Health Center; additional certification and educational requirements may apply to specific programs within the center.

  • Possess a valid Indiana driver's license that meets Eskenazi Health's driving policy requirements.

  • Ability to meet Indiana Medicaid billing requirements as an Other Behavioral Health Professional (OBHP) or Qualified Behavioral Health Professional (QBHP) as defined by Indiana Medicaid regulations.





Essential Functions and Responsibilities






  • Engage and involve the PARC participant, family members, and loved ones of the participant in PARC programming.

  • Collaborate with the treatment team to develop, revise, and implement treatment plans, assessments, and client evaluations in a timely, accurate, and high-quality manner using Shared Decision Making.

  • Exercise discretion and independent judgment to create treatment plans, consulting with the treatment team for guidance and input.

  • Advocate for clients to facilitate their access to necessary services, actively engaging in building community partnerships and resources.

  • Implement treatment plans and coordinate service adjustments based on the established plans.

  • Identify and report any high-risk patient symptoms or circumstances that may pose a risk of self-harm or harm to others, seeking consultation from higher-level clinical staff when needed.

  • Complete relevant legal documentation as required by the treatment plan, ensuring compliance with established protocols.

  • Interpret and apply service and billing procedures according to client needs, ensuring accurate and timely billing practices.

  • Collaborate with family members, significant others, and community supports to assist clients in achieving their goals effectively.

  • Adapt activities and care practices to meet the age-specific needs of clients served.

  • Provide quality case management and activities of daily living skills training to clients, monitoring their progress and documenting it accurately.

  • Meet agency standards for the timely completion of progress notes and client service hours.

  • Demonstrate a commitment to professional growth by actively engaging in clinical supervision and adhering to the Recovery Model principles, including AIDET, motivational interviewing, and accountability levels.

  • Completes data collection to track outcomes in a timely manner.

  • Supports PARC participants using crisis intervention techniques.





Knowledge, Skills & Abilities




Knowledge:


  • Knowledge of best standards for age-appropriate care.

  • Demonstrate knowledge of the principles of growth and development over the life span.

  • Training in the use and side effects of psychotropic medications.

  • Ability to understand and apply motivational interviewing to deliver targeted interventions.

Skills:


  • Advanced critical thinking regarding assessment, crisis intervention, within a strength-based framework.

  • Strong interpersonal skills/ability to communicate clearly and concisely in both verbal and written communication.

  • Practice in mental health and addictions treatment.

  • Experience in providing risk assessments.

  • Demonstrates flexibility and openness to learning.

  • Have an interest in innovative treatments and research.

  • Skilled at working with youth and young adults.

Abilities:


  • Ability to gather, interpret, and analyze client data.

  • Ability to provide case presentations to AIC nursing and paramedic staff.

  • Ability and openness to receive feedback.
