Qualifications & Requirements:

• 1 year as Case Manager experience required and must have 2 years of acute care hospital experience...
  
  
  Predominantly discharge planning (for example facilitating DME, transport, next site of care(home health, rehab, home hospice, wound care, infusion)
  
  
  Will float between MS, ER, ICU, and Medical Psychology...
