Description: Our client is currently seeking a Social Services - LMSW in Marlton, NJ!



Job Details: On-going Contract (W2, Weekly Pay, Benefits Available)



Location: **ON-SITE** - Marlton, NJ 08053



Hours: Monday-Friday 8:00AM-4:30PM or 8:30AM-5:00PM



Compensation: Competitive and based on experience

Floating based on needs, required to float between 7 hospitals (Voorhees, Marlton, Mt. Holly, Willingboro, OLOL, Camden Satellite, Berlin Satellite)*

This job will have the following responsibilities:

• Care Coordination: completes appropriate assessments and plans effectively to meet patient needs, monitors the length of stay and promotes efficient utilization of resources.


• Acts as a clinical expert and maintains ongoing knowledge of clinical practice guidelines.


• Engages the physician and builds seamless continuity of care and is the physicians single consistent resource.


• Problem resolution, patient/family communication.


• Helps identify factors impeding patient progression, resolves, escalates and documents as appropriate.


• Refers appropriate cases for Social Work intervention...
  
  

Qualifications & Requirements:

• Masters prepared Social Worker
  
  


• RN or LMSW License Required
  
  


• -Epic experience required
  
  


• Minimum 1-2 years of acute care experience and/or case management in acute/SAR setting
  
  


• COVID Vaccine + Booster
  
  


• Flu Shot
