Job Details:


Title: Nurse Practitioner

Location: Bronx, NY

Type of Engagement: Contract

Rate: Competitive and Dependent on Experience



Responsibilities:


1. Conduct comprehensive assignments, patient facing

2 Effectively manage acute and chronic medical and behavioral conditions

3.Conduct advanced illness and advanced care planning conversations

4. Provide patients and caregivers with counseling and education

5. Communicate and collaborate with the interdisciplinary care team



Required Qualifications:

1. Board certified through the American Academy of Nurse Practitioners

2. Graduate of an accredited Master of Science Nursing program

Monday-Friday 8-5
