Job Description:

Centers for Behavioral Health LLC serves individuals with mental illness in the community. We strive to instill hope, increase independence and promote recovery and wellness, through advocacy and education. Our onsite life skills program offers clients a safe, supportive and structured environment to participate in groups, activities and community outings. Our skill-based groups and outings promote socialization, links to the community resources, independent living skills, health and wellness, as well as recovery and resiliency.

If you are dedicated to assisting others achieve goals, join our team of compassionate coordinators who help organize, coordinate and provide support to those with mental health diagnoses.

Responsibilities include:
Providing and creating a safe welcoming space for clients
Providing case management services to assigned client caseload
Assessing clients needs, abilities, and strengths using person-centered planning
Assisting clients in development, implementation and review of goals for their rehabilitation plan
Providing community linkage for resources and referrals
Maintaining contact and collaboration with family and community providers, with client consent
Ability to facilitate psychoeducational groups, both in person and virtually
Opportunity to learn and gain experience working with the mentally ill adult population
Assisting clients in appointment management
Coordinating transition and discharge planning with clients
Maintaining objective, accurate, and timely documentation in client record in accordance to governing bodies regulations, standards and timeframes.
Maintaining client confidentiality and protect private information in accordance with policy
Providing advocacy for client rights
Utilizing computer software to maintain electronic client records
Providing psychiatric rehabilitation services that include planning and leading groups, activities and community outings
Provides crisis/emergency intervention as necessary
Operating agency vehicle safely and efficiently to transport clients as necessary

Minimum Qualifications:
Minimum of a high school diploma with experience working in the mental health field OR,
Bachelors degree in a mental health related field
Ability to operate a computer and basic office equipment
Answer a telephone and communicate in a pleasant and helpful manner
Read, understand and follow oral and written instructions,
Establish and maintain effective working relationship with clients, employees and the public.
Must be well organized

Physical Demands: Requires sitting and standing associated with a normal office environment. Manual dexterity is needed for using a phone and computer keyboard. Must be able to lift up to 30lbs.
