Job Description:

Position eligible for $4,000 sign-on bonus

Primary Job Purpose: To ensure the accuracy of MDS’s and the timely transmission of such data to the State of Colorado. Provide education to care partners relative to MDS.

Essential Job Functions:


  • Practice the Values, the Mission, and Philosophy of Eben Ezer Lutheran Care Center.


  • Enters MDS information into the electronic medical record system.

  • Reviews MDS’s for completion and accuracy and transmits such information to the State of Colorado on a regular basis.

  • Coordinators, schedules, monitors for completion of all MDS’s, & CAA’s.
    • Including interview assessments of neighbors

  • Responsible for scheduling Care Conferences with the assistance of Social Workers.

  • Responsible for updating care plans.

  • Responsible for updating the MDS with all discharges, readmissions, deaths, and monitor UDS (User Defined Assessment) completion.

  • Responsible for completing Medicare A Certifications and monitoring schedule for physician signature.

  • Responsible for Medicare A assessment accuracy, review and completion.

  • Responsiblefor weekly MDS quality committee

  • Responsible for section G.G. MDS review daily or as needed with IDT team.

  • Assists with defining significant changes in neighbors.

  • Responsible for leading, preparing and communication related to Medicare A team weekly meeting.

  • Participates in the following committees; Huddle, CQI, Triple Check, Wound Team, care conferences, Continued Stay review for Secured neighborhood, MDS QM monthly review, nurse education and monthly meetings and others as needed.

  • Review PPS billing report for accuracy and billings

  • Updates 802 in the absence of the NHA or DON.

  • Review’s QI Indicator reports monthly and as needed.

  • Assists team members with researching items as needed.

  • Must maintain confidentiality of all work related information according to HIPPA regulations.

  • Acts as a resource to other staff providing PPS & MDS information as needed.

  • Reviews potential admissions as part of the admissions team

  • Assists with compiling and reviewing the 24-hour nursing report in the absence of a nursing supervisor.

  • Assists with nursing care as directed by the D.O.N.

  • Participates in discharge planning as directed by the Director of Nursing.

  • Maintains current knowledge of state and federal regulations, and MDS RAI manual and changes to MDS.

  • Assist the D.O.N. with setting nursing goals and objectives, policies and procedures, and quality assurance measures.

  • In addition to the responsibilities described above, the role may include other responsibilities and duties as assigned on occasion, based upon Eben Ezer needs or requirements.

Qualifications, Education, and Experience:


  • Registered Nurse

  • AANAC Certification

  • 1-2 years experience working in the Long Term Care field

  • 1 year experience in nursing assessments

  • 1 year experience/training with MDS

  • Knowledge of Medicare rules and regulations

Non-Essential Job Functions:


  • Participates in other meetings and committees as mentioned under Essential Duties, including the Quality and Risk Management Team, Nurses Meetings, and the Wound Team.

Material and Equipment used, but not limited to:


  • Ability to perform basic computer functions and to learn new programs as used by the facility.

  • Equipment associated with nursing.

Working Environment/Physical Activities:


  • Must be able to stand and walk 5-7 hours a day.

  • On office days, must be able to sit up to 4-8 hours.

  • Must be able to lift and carry 0-50 pounds by self regularly and 50+ pounds occasionally with assistance.

  • Occasionally will lift neighbors with assistance.

  • Position frequently requires bending, stooping, kneeling, reaching, handling, and keyboarding.

Working Hours:


  • As set by the Administrator and or D.O.N.

  • Rotates as on-call clinician with Director of Nursing

Working Location:


  • This is not a remote position and work must be performed in-person.
