The Care Coordinator functions as an integral part of the Clinical office staff and is responsible for coordinating the interdisciplinary care for a specific patient population by collaborating with administrative and clinical staff, physicians, and patients and their support groups. This position may incorporate the following roles, including but not limited to: direct patient care provider, advocate, educator, consultant, researcher, and quality manager. These duties shall be carried out in accordance with the Mission of University of Cincinnati Physicians, Inc. and the College of Medicine.Minimum Required: Bachelor's Degree in Nursing required. | Current active Nursing license in the State of Ohio. | A minimum of two years experience with specific patient population required; a minimum of one year office experience preferred.PATIENT POPULATION - (CLINICAL ONLY):

Engages in population appropriate communication.

Has knowledge of growth and development milestones and tasks.

Gives clear instructions to patients/family regarding treatment.

Involves family/guardian in the assessment, initial treatment and continuing care of the patient.

Identifies any physical limitations of the patient and deploys intervention when necessary.

Recognizes and responds appropriately to patients/families with behavioral health problems.

Interprets population related data and plans care appropriately.

Identifies and responds appropriately to different needs resulting from, unique psychological needs or those associated with religious / cultural norms. Performs treatments, administers medication or operates equipment safely.

Recognizes and responds to signs/symptoms of abuse or neglect.

Assist in the assessment, evaluation, planning, and implementation of age-appropriate care for a specific patient population including provisions of direct patient care as part of the clinical team.

Collaborate with multi-disciplinary professionals to facilitate continuity of care, including but not limited to home health agencies, referring physicians, social support services, and other community support services.

Maintain excellent relations with physicians practicing in the clinical area while adhering to the professional and legal standard of nursing practice.

Assess for and coordinate teaching plan encompassing patient and support group needs for patient education related to risk reduction/prevention, disease state and optimal health promotion 9plan of care) to include but not limited to patient/support group readiness to learn, barriers to learning and preferred method of learning. Implement and evaluate effectiveness of teaching plan.

Act as clinical resource or consultant for specific patient population with regard to condition and age, including participation in telephone triage.

Collaborate with physicians, management and other health professionals in the development and review of evidence-based standards of care. Reviews and revises standards as necessary with implementation following approval by physicians and division Medical Director.

Assists the physician in documenting notes and in doing orders and prescriptions by setting them up in the Electronic Medical Record. All documents, prescriptions, and orders will be pended to the physician to review, edit, and sign as appropriate.

Provide professional, educational and clinical mentoring/resourcing to clerical and clinical staff, health care students per contract guidelines), other departments and external agencies as needed.

Actively cross-train into related departments to provide required support as needed.

Coordinate ancillary services such as diagnostic testing/imaging, hospital procedures and/or admission, additional patient visits, and acquisition of pertinent medical records. This includes necessary patient education, test result follow up with patient and physicians, and ongoing management as required.

Participate in division meetings to enhance communication, planning, problem-solving and sharing of resources in support of center operations, departmental and agency goals.

Participate in research activities as required, including but not limited to collecting and analyzing data, collaborating with improvement teams and communicating results of quality improvement initiatives.

Maintain positive, professional and cooperative demeanor throughout all interactions with patients, staff, physicians and members of management.

Maintain strict confidentiality.

Reports directly to the Clinical Supervisor, Clinical Operations and follows appropriate chain of command in regards to all issues regarding clinical department.
