Job Summary:

The Reentry Outreach Case Manager is responsible for providing outreach and assisting in coordination of reentry and recovery services in Washington County Maryland. In addition to managing a participant's case load, he/she will help to coordinate with community providers the service needs of participants. they will work alongside the Director of Workforce and Human Services and Reentry team to develop and implement networking strategies to better connect services and participants in Washington County Maryland.

ESSENTIAL FUNCTIONS: 


  1. Act as liaison between assigned clients, referral and funding sources. 

  2. Participate in the development and implementation of case finding and client recruitment initiatives.  

  3. Perform screening, intake and orientation functions with applicants in the region to ensure compliance with Housing and Urban Development, Homeless Management Information Systems, and Coordinated Entry requirements. And to facilitate client/exit entrance into appropriate programs and services.  

  4. Act as case manager for assigned clients in reentry or recovery and provide information about and referral to services needed through community resources (i.e.: adult education classes, childcare, transportation, employment, credit counseling, transitional, rapid rehousing, mental/somatic health care).  

  5. Provide employment and housing support and assist clients to identify and utilize natural support systems. 

  6. Educate clients, community organizations and employers concerning HGI programs and services. 

  7. Act as an advocate for assigned clients.  

  8. Cooperate and maintain good rapport with clients, employers, staff and referring agencies. 

  9. Maintain all relevant client records, case notes and other documentation including attendance records and progress reports. Responsible for additional auditing and monitoring of client database system, and Paycom 

  10. Complete monthly progress reports and submit fee-for-service invoices as directed. 

  11. Assist with program evaluation and community needs assessment to identify program and service needs.  

  12. Participate in development and submission of program and service proposals to expand and diversify program funding as directed.  

  13. Participate in agency meetings and committees as directed.  

  14. Monitor and assess client’s human services and housing needs and make recommendations to funding sources for future programming.  

  15. Document client’s progress to determine appropriate community-based referrals needed.  

  16. Develop volunteer relationships in the community and maintain relationships through consistent contact and documentation 

  17.  

    ADDITONAL FUNCTIONS TO BE COMPLETED TOGETHER WITH DIRECTOR OF WORKFORCE and HUMAN SERVICES: 


  18. Act as liaison with local service providers, referral and funding sources and cultivate positive working relationships in the communities and where personnel and programs may be supervised.  

  19. Complete and submit quarterly reports and program proposals to funding source(s) as directed.  

  20. Assist in the coordination of Reentry deliverables

  21. Identification and documentation educational, employment and housing barriers. 

  22. Assessment and documentation of services needed.  

  23. Development, documentation, and implementation of individualized program plans. 

  24. Quality assurance of client documentation. 

  25. Along with entire Workforce Development and Human Services team, facilitate cooperative relationships between operations departments to ensure quality employment and training service provision and maximize client productivity through weekly meetings and regular contact.  

  26. Produce and provide required reports for funding sources.  

  27. Participate in trainings to develop professional and management skills, which includes maintenance of all necessary professional credentials.  

  28. Assist with the preparation of HGI for CARF Compliance as well as other applicable licensures and accreditations. 

  29. Other duties as assigned by the Director of Workforce and Human Services  and/or Chief Mission Officer. 

Education and Experience


  1. Ability to learn and apply HGI’s policies and procedures as well as attend required training.  

  2. Strong oral and written communication skills. 

  3. Ability to convey a positive and professional image to clients, staff, and related agencies. 

  4. Ability to provide motivation, leadership, guidance, and supervision to assigned clients. 

  5. Ability to relate to and have an understanding of the needs of people with disabling and disadvantaging conditions. 

  6. Ability to maintain composure and problem solve in high pressure situations

  7. Ability to think out side of the box and develop creative solutions to challenges faced by homeless program participants in Washington County

  8. Valid driver’s license, clean driving record and provide own reliable transportation. 

  9. Ability to work flexible hours, meet deadlines, and attend required meetings and trainings.  

  10. Ability to maintain confidential information. 

 

EDUCATION AND EXPERIENCE: 


  1. Experience with public assistance system preferred, as well as working with state and federal funding source

  2. Lived reentry or recovery experience preferred

  3. 3-5 years Experience in Case Management roll
