Job Description:

JOB SUMMARY: The CET Care Coordinator works closely and collaboratively with BestCare's ED Peer Support Specialists, Certified Recovery Mentors and St. Charles Health Systems to identify and engage hospital patients with substance use disorders to connect them with appropriate services in the community to reduce their chances of readmission to the hospital or emergency department and improve their experience and outcomes using the Intentional Peer Support model.

ESSENTIAL FUNCTIONS:

Works closely and collaboratively with St. Charles Health Systems staff to improve the patients' experience of care, improve the cost of care, and improve the work life of health care providers;
Provides outreach, engagement, assessment, and care coordination services to identified hospital patients with substance use disorders, with the goal of reducing unnecessary readmissions into the hospital or emergency department;
Provides services within the framework of the Intentional Peer Support model;
Works closely with community-based SUD providers and other social service providers, to support rapid access to appropriate SUD treatment upon discharge from the hospital;
Provides ongoing support to identified patients with SUD, when appropriate, during the hospital stay, post-discharge, and during any readmissions;
Maintains care documentation in the hospital medical record and BestCare's EMR system;
Maintains data on patient contacts, engagement in SUD services, continued contacts post-discharge, and re-admissions to either the hospital or the emergency department;
Models ethical, compassionate, and professional behavior while in the work setting;
Provides clinical supervision/documentation, direction, and support to the hospital-based Certified Recovery Mentors and Peer Support Specialists;
Provides clinical supervision/documentation and direction and support to DOPT Certified Recovery Mentors;
Provides written and in-person reports on the hospital-based services as necessary;
Other related duties as assigned.

QUALIFICATIONS:

EDUCATION AND/OR EXPERIENCE REQUIREMENTS:

High School Diploma (or equivalent)
2 years of experience working in substance use disorder or other behavioral health setting
For individuals in recovery from substance-use disorder, must have maintained continuous recovery for the previous two years at the time of hire


LICENSES AND CERTIFICATIONS:

QMHA-I or QMHA-R with ability/commitment to obtain within 6 months required, or CADC-I required

Must maintain a valid Oregon Driver License or ability to obtain one upon hire, and be insurable under the organization's auto liability coverage policy (minimum 21 years of age and with no Type A violations in the past 3 years, or three (3) or more Type B violations)


PREFERRED:

Associate's degree in addiction a plus
Bilingual in English/Spanish a plus


REQUIRED COMPETENCIES: Must have demonstrated competency or ability to attain competency for each of the following within a reasonable period:

Demonstrated knowledge and ability to provide SUD treatment including client evaluation and individual, group, family and other counseling techniques;
Proficiency with use of ASAM criteria in determining length of stay for clients;
Strong understanding and application of the "Stages of Change/Motivational Interviewing" and "Intentional Peer Support" models of treatment;
Demonstrated ability to network with all resources to provide the best customer service;
Proficiency in MS Office 365 (Word, Excel, Outlook), EHR platform, databases, virtual meeting platforms, internet, and ability to learn new or updated software;
Strong interpersonal and customer service skills;
Strong communication skills (oral and written);
Ability to work effectively and respectfully in a diverse, multi-cultural environment;
Strong organizational skills and attention to detail, accuracy, and follow-through;
Excellent time management skills with a proven ability to meet deadlines;
Critical thinking skills;
Ability to maintain strict confidence as required by HIPAA, 42 CFR, and Oregon Statutes;
Ability to build and maintain positive relationships;
Ability to drive motor vehicle to perform job;
Ability to function well and use good judgment in a high-paced and at times stressful environment;
Ability to manage conflict resolution and anger/fear/hostility/violence of others appropriately and effectively;
Ability to work independently as well as participating as a positive, collaborative team member.


TOOLS AND EQUIPMENT USED: Standard office equipment including computer/laptop, copy/fax/scanning machine, printer, telephone or multi-line phone.

Must wear attire as prescribed for staff working in the assigned Program
Frequently sits at a desk, stands, walks, bends/stoops, reaches above/below shoulders, uses arms, wrists, hands, and grasps/squeezes
Occasionally twists, crouches/squats
Must be able to perform repetitive tasks/motions, have good manual dexterity
Must be able to lift/carry up to 20 lbs. frequently and push/pull 21-75 pounds occasionally
Must be able to hear alarms/telephone/audio/normal speaking voice
Must have clarity of vision: Near (<20"); Mid (>20" to < 20'); Far (> 20')
Must be able to drive motor vehicle to perform job
May be exposed to communicable disease
