About our Team:

We employ a facility-based care model. We have an extensive referral network which will allow us to provide care for more patients. You will go above and beyond to connect with patients in all the markets that are eligible for RPM and CCM programs in a non-judgmental, respectful and effective way. You will work with a multi-disciplinary team that consists of providers, Registered nurses, coordinators, medical assistants, and operations support.
