The Recovery Care Coordinator will provide daily efforts in the areas of strategic outreach, patient recruitment, patient retention, primary care coordination, specialty care coordination; follow up tracking, community outreach and program evaluation. The position will be a key role in the implementation of integrated and coordinated substance abuse services for CHCI patients, and will be the key personnel responsible for the internal and external communication with the providers, staff and patients at their assigned sites.



ROLE AND RESPONSIBILITIES



  • Conduct strategic outreach to all CHCI sites participating in Substance Abuse expansion and collaborate on recruitment of new patients into medication-assisted treatment for opioid dependency.



  • Conduct external outreach to community partners in an effort to raise awareness about the program and services available at CHCI, specifically substance abuse expansion.



  • Actively recruit patients for participation in MAT Program and ensure that all sites have appropriate consent forms and disclosures for patient participation in program and research.

  • Work with Project ECHO providers on site to ensure that all patients enrolled in SA expansion services are entered into CHCI database and appropriately flagged in CHC EHR.

  • Ensure the detail, accuracy and timeliness of data entry and reporting.

  • Work in collaboration with SA expansion team to develop and implement retention strategies for patients in the program.

  • Work with Project Director to ensure the progress of all outcome measures and performance improvement projects with relationship to SA expansion grant requirements.

  • Participate in community events, which will promote SA expansion services and raise awareness about all services at CHC.

  • Work with Project Director and SA Expansion team on specific evaluation goals.

  • Actively participate in all CKP, Interdisciplinary and Quality Improvement Meetings on a regular basis.

  • Identify and participate in trainings as appropriate for the professional development and accomplishment of all program goals.




QUALIFICATIONS



Bachelors in related field


Experience in outreach and care coordination


Experience in substance abuse field


Ability to communicate effectively with hard to reach populations

  • Bi-lingual English/Spanish

  • Certified Alcohol and Drug Counselor
