• Develops and implements plans of care for specific patient populations. Coordinates and collaborates with multi-disciplinary team members to facilitate integrated and comprehensive care. Provides support to healthcare professionals and patients in relation to dysrhythmias, implanted devices and invasive procedures. Responsible for monitoring rhythm and documenting the device settings, and the rhythm response to the therapy, this includes but not limited to interrogating and making adjustments to the device per the physician's orders.
• CLINICAL JUDGMENT related to implantable devices, Trasntelephinic monitoring (TTM) and cardiac resynchronization. SYSTEMS THINKING CLINICAL INQUIRY UNIT SPECIFIC RESPONSIBILITIES within the Device Clinic including the Cardiovascular department


1. CLINICAL JUDGMENT related to implantable devices, Trasntelephinic monitoring (TTM) and cardiac resynchronization.
a. Appropriately assesses, plans, implements and evaluates patient care, specific to population served, including but not limited to initiating action plans in relation to fail pacemaker systems i.e. battery depletion, lead failure, or dislodgement.
b. Follows policies/procedures, clinical practice standards pertinent to care, including emergency situations
c. Matches formal knowledge with clinical events to make decisions and provide effective care related to implantable devices including but not limited to post procedure injury like infection, pneumo/hemothorax.
d. Prioritizes and organizes work appropriately to effectively meet patient needs
e. Appropriately delegates to and supervises team members based on assessment of competencies, within scope of practice to ensure appropriate implementation of care. Collaborates & communicates relevant patient data to appropriate persons clearly and effectively, both orally and in writing (e.g. handoffs)
f. Effectively develops teaching plan and regularly integrates teaching related to implantable devices into patient care


2. SYSTEMS THINKING
. Recognizes available resources/tools for meeting patient/family needs
a. Completes assignments in an efficient and timely manner, requesting and utilizing resources appropriately
b. Documents patient assessment, plan, interventions and response in accordance with applicable policies and procedures 2.4 Communicates with Nurse Manager/ or designee about availability and management of supplies, system equipment and personnel resources at unit/department level. Assists with the inventory and/or ordering of supplies needed for implantable devices.
c. Demonstrates contribution to unit-based programs and strategies to monitor and continuously improve the quality of patient care. Including but not limited to participating in policy and procedure development and implementation, maintaining a database for implant Recalls and Follow-ups, providing monthly reimbursement reports, and providing test error reports, identifies opportunities for improvement in processes of care and other improvements at a unit level.


3. CLINICAL INQUIRY
. Shows commitment to learning new knowledge and skills to enhance service to patients/families and achieve organizational goals validated by implementing and validating practices based upon current nursing evidence, including but not limited to uses knowledge to prolong life of generators, increasing the interval between implants.
a. Advances the profession by assisting in clinical education and orientation of other staff and students; may function as preceptor for staff.
b. Uses available resources to answer questions in regards to best practices, reflects upon practices on an on going basis


4. UNIT SPECIFIC RESPONSIBILITIES within the Device Clinic including the Cardiovascular department
. Provides clinical training/education to clinical staff within the organization, this training/education is related to implantable devices, Transtelephonic monitoring (TTM) and cardiac resynchronization.
a. Articulates Nusing Sensitive Indicators and quality goals for unit and describes role in accomplishing expectations; describes role and participates in quality improvement initiatives.
b. Monitors revenue cycle components including coding, charges, and billing reconciliation and corrections.
c. Processes interrogations within the device clinic in order to facilitate quality test results which encompasses downloading electronically recorded tracings into management systems, maintaining report databases for provider comparison, entering provider interpretation changes into database management systems, generating random reports and distributing these reports to nursing units and provider offices within 72 hours.
d. Evaluates all interrogations on patients in order to ensure quality tracings which encompasses reviewing these tracings for technical accuracy, correct patient information and procedural proficiency, maintaining a problem testing log and reporting error rates on a monthly basis, and discussing clinical evaluation issues with providers.
e. Oversees remote monitoring and performs Transtelephonic monitoring when necessary and reports issues and/or concerns to the leadership team.
f. Organizes, implements and completes projects and/or tasks as appropriately assigned.



Qualifications

Experience Minimum Required

• At least 2 years of cardiovascular experience or critical care experience



Experience Preferred/Desired

• 2 years experience in a Device Clinic or EP lab



Education Minimum Required

• Registered Nursing License in NH or bachelor's in health science



Education Preferred/Desired

• BSN or enrollment in BSN program.



Training Minimum Required

• Understanding of Pacemakers, ICD's, BiV and Loop recorder devices
Training Preferred/Desired
• Experience with interrogation and interpretation of Pacermakers, ICD's, BiV, and Loop recorder devices.



Special Skills Minimum Required

• Advanced computer skills including but not limited to MS Office programs and database management programs. Excellent organization, interpersonal, communication and conflict management skills.



Special Skills Preferred/Desired
• Experience with device programmers



Licensure and/or Certifications Required

• BLS



Licensure and/or Certifications Preferred/Desired

• RCES
