Job Description:

Healthcare provider seeking a dynamic individual who is focused on making a difference, to join our caring and diverse team in delivering loving and compassionate care to our campus community in the role of Activities Coordinator.

Primary Job Purpose: Assessing and assuring an ongoing program of meaningful and appropriate quality services and activities that are available for our Neighbors based upon individual comprehensive assessments.

Customer Service: Adherence to the MOSES program to include Mission, Ownership, Success, Excellence, and Service.

Essential Job Functions:


  • Practices the Values, the Mission, and Philosophy of Eben Ezer Lutheran Care Center.

  • Provide opportunities for recreation, work, education, worship, self-maintenance, and social and interpersonal relationships for neighbors, while encouraging their participation, independence, and self-expression.

  • Plan, organize, schedule, and monitor individual and group activities and special programs (such as crafts, games, trips, parties, and the like for neighbors).

  • Research and implement new ideas, activities, and programs in accordance with neighbors needs based on their comprehensive assessments, interest backgrounds and ability to participate.

  • Provide meaningful activities for persons with dementia by modifying activities based on ability, strengths, interests and life experiences.

  • Inform and update neighbors and staff as to regular activities and special programs.

  • Develop effective communication with administration and with other department heads, and coordinate the program with other nursing home services.

  • Provide individual lifestyle profiles, activity assessments, and ongoing program evaluation.

  • Assemble and utilize all equipment within the community kitchen for special events.

  • Provide proper documentation and evaluations of care plans to ensure activity goals are met.

  • Maintain activity areas, equipment, and supplies in a safe, clean, and orderly manner and keeping inventory of and ordering necessary materials.

  • Determine proper size of groups based on activity and the participants ability using techniques to keep people engaged in participation and understanding the importance of reducing distractions.

  • Utilize team building skills.

  • Attend related meetings, conferences, and tours.

  • Advise and assist Resident Council.

  • Assist with family group activities and various community functions.

  • Drive the community bus for special excursions related to recreational activities.

  • Maintain punctuality when reporting to work and leaving work.

  • Development of Care Plans as well as a working knowledge of CAAs and the MDS process.

  • Completes required reports timely and accurately.

  • Must maintain confidentiality of all work-related information according to HIPAA regulations.

  • Must be able to push/pull up to 250 pounds regularly using a wheelchair. Must be able to lift 0-50 pounds regularly by self and 50+ pounds occasionally with assistance.

  • In addition to the responsibilities described above, the role may include other responsibilities and duties as assigned on occasion, based upon Eben Ezer needs or requirements.

Qualifications: To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. In addition, qualified individual must be at least 21 years old and be insurable by our auto insurance carrier. The requirements listed below are representative of the knowledge, skill, and/or ability required. Must possess a valid Colorado driver’s license. Must follow transportation procedures, including use of seatbelt by driver.

  • Understanding of mechanical functioning of body (in order to prevent aggravation of and to maintain/improve Neighbors’ physical condition), as well as basic knowledge of physical exercise and familiarity with medical terminology.

Education and Experience: High School Diploma or equivalent. Successful full-time experience in related setting, gerontology background and/or work experience with other special populations. Knowledge of aspects of aging preferred.

Required Experience/Skills: Successful full-time experience in related setting, geriatrics/gerontology background, and/or work experience with other special populations, and knowledge of aging required. Activity skills and working knowledge of a variety of recreational activities preferred. Competence preferred in arts and crafts, table and card games, exercises, and various hobbies. Ability to work with a Windows operating system and word processing programs. Ability to learn new programs as used by the community.

Material and Equipment used, but not limited to: Computer, phone, copier, fax, calculator, shredder, kitchen equipment, and facility van or bus. Pushing wheelchairs, supply carts, and activity equipment. Assembly, use and cleaning of popcorn machine and other such equipment.

Working Environment/Physical Activities: Working in an office as well as throughout the community including Neighbor’s rooms. Employee occasionally sits, frequently stands, bends, walks and reaches during working hours. Employee frequently pushes neighbors in wheelchairs, as well as carts, tables, and various items related to activities. Employee is subject to physically ill neighbors as well as hostile and emotionally upset neighbors and/or families.

Working Hours: Position requires working M-F, weekends, and holidays as scheduled. Schedule may vary, based on needs and supervisor discretion.

Working Location: This is not a remote position and work must be performed in person.
