Legacy Silverton Medical Center serves the heart of the Willamette Valley with a wide mix of services, many not typically available in a community hospital: CT scan (computerized tomography), nuclear medicine, echocardiography, family birth center and much more.
RESPONSIBILITIES:
Interviews, assesses and evaluates patients and families who present for mental health; provides professional counseling and treatment with the goal of maximizing personal/familial functioning.
GENERAL ACCOUNTABILITIES AND ESSENTIAL FUNCTIONS:
Interview, assess and evaluate clients and families; provide professional counseling and treatment with the goal of maximizing personal/family stability
Prepare intake summaries, treatment plans and case summaries
Chart members’ treatment and progress in accordance with state and other regulations and in keeping with accepted community standards
Develop, implement and evaluate treatment approaches for clients and affected family members, including individual, family, group or intensive therapeutic modalities
Confer and consult with center physicians regarding client's progress and treatment
Utilizes resources within the medical home that meet the needs of the client's treatment goals
Interviews patients and family members to assess nature and extent of psychiatric problem
Develops implements and evaluates treatment approaches for patients which may include individual, marital, and/or family therapeutic modalities
Provides team treatment consultation and approaches within mental health department.
Maintains confidential individual case summaries
Consults with and accepts guidance from Mental Health clinical director for complex patient interventions and development of plan of care
Works in collaborative team model to further strategic departmental goals and objectives
EDUCATION:
Master of Social Work degree from an accredited School of Social Work required.
EXPERIENCE:
This position requires a minimum of one year relevant experience. Experience in one or more of the following areas strongly preferred:
Care Management
Behavioral Health
Social Services
Addictions treatment
