Job Description:

We are searching for a Behavioral Health Social Worker for the Department of Psychology -- someone who works well in a fast-paced setting and is committed to supporting the Foster Care Clinic and Psychology patients with acute care needs. This professional will serve as a social worker and an ambulatory service representative, facilitating communication among Child Protective Services (CPS), attorneys, law enforcement, and foster care parents, while efficiently coordinating therapy appointments for children. Beyond these responsibilities, the individual will lead the transformation of workflow processes by transitioning the appointment scheduling work queue, managing secure communication through video links, ensuring updated patient care details, and offering foster care families appropriate access to patient information. The person in this position will also assist with crisis support for non-foster families (e.g., locating and transitioning patients with escalating needs to higher levels of care, such as inpatient behavioral health, IOP, PHP, and Residential Treatment programs).

As the volume, acuity, and complexity of patients seen by Psychology providers increase, we need an additional social worker to receive warm handoffs for referrals and transfers to specialized and higher levels of care, inside and outside of our system. Some families will require case management services, including ongoing interactions with a family as they navigate the broader behavioral health and child welfare systems. This role will apply optimal utilization of health care services and community resources in accordance with the principles of human growth and development. The social worker will assess, plan, implement, deliver, monitor, and evaluate the options and services required to address a patient's individualized care plan in collaboration with the interdisciplinary team to achieve discharge goals.
