Mental Health Case Manager / Skills Trainer at BestCare Treatment Services, Inc

Job Description:

JOB SUMMARY: This position includes a combination of care coordination, data management, and direct clinical duties. The Case Manager is responsible for coordinating services and benefits for all individuals admitted to the crisis stabilization program and must develop strong relationships with clients and community providers. The Case Manager is responsible for data management, including tracking compliance with contract metrics and program outcomes, and will provide direct clinical services, including periodic substance use assessments, to promote crisis stabilization.



ESSENTIAL FUNCTIONS:

Works closely with other staff as a member of a treatment team in a key role on the Mental Health Team;
Assists clients with accessing resources, benefits, and entitlement programs;
Provides support to mental health clients in achieving their treatment plan (ISSP/Individual Services & Support Plan) goals and objectives;
Develops and maintains strong collaborative relationships with a wide range of community partners, including (but not limited to): Self-Sufficiency and Seniors/People with Disabilities, the County Adult Community Justice, Public Health, Mosaic Medical and other primary care clinics including dental. (Note: There is a strong emphasis on helping coordinate primary and dental care for mental health clients with severe and persistent mental illness or serious mental illness and effective collaboration and bridging between behavioral health and primary care);
Assists clients who have been identified to be in the ACT-Assertive Community Treatment program with completing their Personal Recovery Plan, under the direction of the mental health clinician;
BestCare Treatment Services as an Agency is a designated Payee for the Social Security Administration. Upon completion of appropriate training, the MH Case Manager may provide Payee services on behalf of BestCare to clients who are approved for Social Security disability;
Provides transportation for clients when appropriate and under the direction of the mental health clinician and/or MH clinical supervisor;
When appropriate, conducts or participates in client home visits and welfare checks, working in conjunction with the mental health clinician and/or MH clinical supervisor. One of the most critical elements of the MH case manager position is staying connected to clients who have been hospitalized and quickly engaging them in follow-up MH services in the community following discharge;
Under the direction of the MH clinician and when appropriate, participates in, co-facilitates, or leads psycho-educational, skill development, anger management, art, other types of groups, when appropriate;
Maintains clinical documentation in a timely manner in client files in the EHR system and in compliance with BestCare timelines and regulatory standards. Documentation should be completed daily whenever possible, but always within 72 hours of provision of the client service. Billing/ encountering is an important component of this clinical position;
Helps facilitate and coordinates services at the BestClub Adult Mental Health Consumer Center. While the position is primarily in relation to and attached to BestClub, the MH Case Manager may be assigned to perform MH case management with mental health clients who are not participants in BestClub;
Participates in training (agency, state, regional, etc.) that will assist the employee in their position;
Models appropriate counselor behavior to the newer staff, and helps train and orient new staff to the program;
In addition to being a positive role model for staff, also provides assistance to clients, demonstrating compassion and being effective in engaging clients in mental health treatment services wherever possible;
Other related duties as assigned.

QUALIFICATIONS:

EDUCATION AND/OR EXPERIENCE REQUIREMENTS:

Bachelor's degree in behavioral science (Psychology, Social Work, Behavioral Sciences, or related field) required or
Minimum of four (4) years of demonstrated work in social services or counseling field required;
Experience working with individuals with severe and persistent mental illness (SPMI)
For individuals in recovery from substance-use disorder, must have maintained continuous recovery for the previous two years at the time of hire;


LICENSES AND CERTIFICATIONS:

QMHA-I required, or QMHA-R with commitment/ability to obtain certification within six (6) months required or
CADC-I required
Must maintain a valid Oregon Driver License or ability to obtain one upon hire, and be insurable under the organization's auto liability coverage policy (minimum 21 years of age and with no Type A violations in the past 3 years, or three (3) or more Type B violations)


PREFERRED:

Bilingual in English/Spanish a plus


REQUIRED COMPETENCIES: Must have demonstrated competency or ability to attain competency for each of the following within a reasonable period:

Demonstrated knowledge and ability to provide treatment of mental health and substance-related disorders including client evaluation and individual, group, family, and other counseling techniques; program policies and procedures for client case management and record keeping;
Ability to electronically record information in client files in the EHR system timely and consistent with BestCare's policies and procedures and applicable OAR's;
Proficiency in MS Office 365 (Word, Excel, Outlook), EHR platform, databases, virtual meeting platforms, internet, and ability to learn new or updated software;
Strong interpersonal and customer service skills;
Strong communication skills (oral and written);
Strong organizational skills and attention to detail, accuracy, and follow-through;
Excellent time management skills with a proven ability to meet deadlines;
Critical thinking skills;
Ability to maintain strict confidence as required by HIPAA, 42 CFR, and Oregon Statutes;
Ability to build and maintain positive relationships;
Ability to function well and use good judgment in a high-paced and at times stressful environment;
Position requires professional and personal skills to cope with stress associated with work involving a high degree of mental, emotional and physical demands.
Ability to manage conflict resolution and anger/fear/hostility/violence of others appropriately and effectively;
Ability to work independently as well as participating as a positive, collaborative team member.




TOOLS AND EQUIPMENT USED: Standard office equipment including computer/laptop, copy/fax/scanning machine, printer, telephone or multi-line phone.
