The Housing Support program is comprised of multiple supports designed to assist individuals living in GHVP-subsidized permanent supportive housing to promote ongoing housing stability. All individuals enrolled in the Housing Support program must receive the following types of support:
Assistance with housing search, leasing, and move-in processes.Purchase of initial household furnishing, deposits, household goods for the one-time move-in needs.Safety and wellness checks and housing safety inspections.Developing a Housing Stability Support Plan as an adjunct to an individuals IRP.Early intervention to mitigate factors impacting housing stability (e.g., late rent payment, lease violations, tenant/landlord or property owner conflicts);Education on the roles, responsibilities, and rights of tenant(s) and the landlord/property owner.Assistance with the annual housing recertification and inspection process.

This program is provided to adults enrolled in GHVP in order to promote housing stability, wellness, independence, recovery, and community integration. Housing stability is measured by ongoing housing and by decreased number of hospitalizations/ER visits/incarcerations, by decreased frequency and duration of crisis episodes, and by increased and/or stable participation in maintenance of personal housing stability and wellness. Support based on the individuals needs is used to promote resiliency while understanding the effects of SPMI and lived trauma. The Housing Support staff will serve as the first point of contact for landlords/property owners for any issues arising with a supportive housing individual and will provide linkage to community; general entitlements; and psychiatric, substance use disorder, medical services, crisis prevention, and intervention services.

Duties & Responsibilities:
Maintain a caseload of individuals who have met the criteria outlined by the TEAM Approach in order to assist with developing a plan of action to transition individuals from the GHVP to other federally funded housing programs. Caseload ratio will be a1:30 as the identified individuals will not require regular face to face meetings based on criteria but will need a minimum of once a month contact in order to prepare for transition to these new programs. This will be a floating caseload in which individuals could reside in all six counties covered by Region 3 and individuals may have to travel to all six counties if necessary. Provide care coordination to ensure stability of housing and independent living skills.Interviews client, family members, and/or support system(s) to assess level of functioning, current Strengths, Needs, Abilities, and Preferences and secure information relative to program requirements for receiving services and recommending appropriate level of care.Participate in weekly treatment team meetings to assist in the development of treatment plans with appropriate therapeutic interventions, notify professionals of any unusual observations or incidents, client progress, and follow up.Develops and maintains a network of community contacts. Facilitates referrals and linkages to community services and communication among clients service providers with VPH to assure a continuum of services.Provide linkage, transport clients, and assist/coach clients in accessing & obtaining community resources.Write precise, accurate, and clear progress notes in our electronic Care Logic system in a timely manner. Maintain required records and information in each clients file. Prepares accurate, complete timely documentation (within 24 hours) of case notes and billing information to meet agency and state standards and provide effective service delivery across a broad range of services.Assist with completing all paperwork necessary to transition an individual to the HCV or HUD811 and must have a strong working knowledge of the transition process. Recognize the need for crisis intervention and notifies supervisor and professional staff of escalated situations.Participate in On-Call Rotation for nights and weekends as a part of the team in order to assist individuals in need of assistance with their housing needs. Meet or exceed Agency Productivity standards.

Program Specific Duties:
Travel in Gwinnett, Rockdale, Newton, DeKalb and Fulton Counties along transporting individuals when needed.Perform certain case management duties.

Minimum Qualifications:
Master's degree in a related field from an accredited college or university OR Bachelor's degree in a related field from an accredited college or university AND Two years of related experience OR One year of experience at the lower level Behavioral Health Counselor 1 (HCP100) or position equivalent.

Note: Some positions may require certification or licensure.

Preferred Qualifications:
Experience working with the Severe and Persistently Mentally ill population along with individuals with co-occurring substance use diagnoses within a community setting or residential setting.Case management experienceExperience working in a high stress, fast-paced environment with a very emotionally and physically changing population.Experience working in multi-disciplinary teams.Ability to multi-task and manage time.Certified Peer Specialist or Certified Addiction Recovery Empowerment Specialist.

Requirements/Competencies:
Must have valid Georgia drivers license and Motor Vehicle Record in accordance with company policy.Candidates for selection must pass a criminal background check (including fingerprinting)Must pass a pre-employment drug screen and subject to random drug screens.Any combination of training and experience, which would have enabled the applicant to acquire the necessary knowledge, skills, and abilities.
