Health Quest and Western Connecticut Health Network have combined to form a new nonprofit health system. The name for the new health system will be Nuvance Health. The new health system was created to provide communities across New York’s Hudson Valley and western Connecticut with more convenient, accessible and affordable care.

Summary: The Case Management Social Worker is responsible for assessment, treatment planning and provision of social work services to patients, families, groups and the interdisciplinary team.

 Job Responsibilities:

• Develops a discharge plan that addresses the psycho-social needs to meet desired goals for the next step in the continuum of care for patients.


• Collaborates with the patient, family or other caregivers, and multidisciplinary team to design a discharge plan respective of the patient’s needs and goals.


• Works as a team with other members of care management, including but not limited to: RN care managers, assistants, coordinators, utilization management staff, and director.


• Facilitates communication among all treatment team members.

 Job Requirements:

• Master Social Worker (MSW) or equivalent experience.


• Minimum of one year experience in Social Work or similar field of practice.


• Current NY State license in Social Work or eligible for limited permit.


• Licensed Clinical Social Worker (LCSW) OR Licensed Master Social Work (LMSW).

Work Schedule: DAY 1

Work Shift: Mon- Friday 8:00AM-4:30PM Alternate 1 weekend/month.

Salary Range: 32.00/hr. – 52.00/hr.
