Job Description:

80% FTE, 32 hours per week, Monday - Friday. No weekends or holidays. Hours may vary based on the operational needs of the department. You will be working at University Hospital in Madison, WI.

We are seeking a Registered Nurse (RN) Navigator to:


  • Work in collaboration with numerous multidisciplinary teams to help navigate patients and families through Pulmonary care.


  • Be a conduit of information between inpatient and outpatient services and pre- and post-surgical care for patients and families.

  • Participate in continuous quality improvement initiatives.

  • Work closely with Pulmonary providers, carry out coordinated plan of care for patients, and be highly organized and great at multi-tasking.

Education:

Minimum – Bachelor of Science – Nursing degree (BSN).

Preferred – Masters degree in Nursing, Social Work, Public Health, Healthcare Administration, or related field.

Work Experience:

Minimum – Three (3) years of relevant clinical nursing experience.

Preferred – One (1) year of relevant experience in the specific patient population being supported.

Licenses and Certifications:

Minimum – Registered Nurse license in the state of Wisconsin and Basic Life Support/CPR.

Preferred – Applicable Oncology certification based on patient population.
