JOB SUMMARY  

This position will report to the Clinical Director, Behavioral Health Services, Virginia and will provide service designed to meet the specific mental health needs of the identified client as they relate to their diagnosis and can be provided in-home, in-office, in-school or other approved/appropriate community setting. Outpatient therapy includes psychoeducational activities/interventions designed to assist the client of the reduction of dysfunction. Services are provided with client-centered and family-centered approaches and are delivered through intensive, time-limited, goal specific interventions. The VASH (Veterans Affairs Supportive Housing) Clinical Case Manager is responsible for the supervision of the case management and clinical services of the families that utilize the VASH Voucher through the VOACC contract with Veteran Affairs. 

 

 

 

REQUIREMENTS 


  • Master's degree in a Mental Health field (e.g., Social Work, Psychology, Counseling). 

  • LPC, LCSW, LMFT (Virginia), with the ability to submit Medicaid claims. 


  • Ability to supervise residents who are registered with the Commonwealth of Virginia Board of Counseling. 

  • Deliver in-home or in-office services. 

  • Provide therapeutic interventions to the identified client in order to enhance and foster the clients optimal functioning and interactions in the home and community. 

  • Continually assess client needs, service availability and appropriateness. 

  • Work cooperatively with treatment team members and the family in the provision of services to meet the goals developed in the Person-Centered Plan. 


  • Provide the appropriate documentation for service delivery including treatment goals, progress notes and billing and submit documentation as specified by Medicaid and VOACC standard. 

  • Provide counseling to individuals or groups regarding psychological or emotional problems, such as stress, substance abuse, or family situations, using evaluative techniques, and develops and implements therapeutic treatment plan. 

  • Interviews patient to obtain information concerning medical history or other pertinent information. 

  • Observes client to detect indications of abnormal physical or mental behavior. 

 

RESPONSIBILITIES 

 


  • Provide therapeutic interventions to identified clients in order to enhance and foster the clients optimal functioning and interactions in the home and community. 

  • Participate in weekly case management and care coordination meeting. 

  • Collaborates with Data Manager on timely submission of practitioner documentation, data outcomes, etc. 


  • Provides or supervises practitioners responsible for crisis counseling interventions. 

  • Continually assess client needs, service availability and appropriateness. 

  • Work cooperatively with treatment team members and the family in the provision of services to meet the goals developed in the Person-Centered Plan. 

  • Provide the appropriate documentation for service delivery including treatment goals, progress notes and billing and submit documentation as specified by Medicaid and VOACC standard 

  • Provide counseling to individuals, groups or families regarding behavioral or emotional problems, such as stress, substance abuse, or family situations, using evaluative techniques, and develops and implements therapeutic treatment plan. 


  • Interview clients to obtain information concerning medical history or other pertinent information. 

  • Observes client to detect indications of abnormal physical or mental behavior. 

  • Collaborate with the executive leadership team members to assist in the development of clinical programs and support the clinical expertise of staff to embed professional development and evidence-based trainings in services across the organization. 

  • Collaborate with Human Resources to contribute to the development and maintenance of the learning management system, which includes the creation of curriculum for learning platforms and materials.   

  • Delivers employee training on organizational excellence and expectations.  


  • Performs facilitation for virtual and in person training programs as needed.

  • The ability to safely operate a motor vehicle to transport oneself, consumers, and program supplies as necessary.

  • Operating office equipment requiring continuous or repetitive hand/arm movements.  

  • The ability to remain in a sitting position for extended periods of time.
