Licensed Practical Nurse



Volunteers of America National Services (VOANS) is seeking a full-time Licensed Practical Nurse (LPN) to join their team in CO! Under the direction and supervision of the Clinical Care Manager and/or Care Team Leader, the Licensed Practical Nurse participates in care planning, executes interventions in accordance with the plan of care and contributes to the evaluation of individualized interventions related to the plan of care. Administers medications, implements primary care orders, and evaluates participant complaints physically and/or by telephone. Provides education to Senior CommUnity Care participant and caregiver in regards to plan of care, nursing treatments or medications. Communicates participant's condition to primary care givers, physician, and care team.


The Licensed Practical Nurse will report to the Clinical Care Manager.


The Licensed Practical Nurse will work onsite at the Senior Community Care of Colorado PACE - Montrose Clinic. Incumbent will be required to make home care visits as needed.



Salary Range: $23.70-$28.60/hr.



$5,000 BONUS



8 or 10 Hour shifts / 4 days per week (2 weeknight's on-call and 1 weekend every 4 weeks)

Essential Functions, Duties & Responsibilities:

Essential Functions:


• Functions as a member of the Care Team. Maintains regular attendance at, and participants in Care Team meetings; communicates participant changes, collaborates on plan of care decisions and coordination for twenty-four (24) hour care delivery. Effectively communicates with team mates during meetings or one-on-one conversations.


• In coordination with the Care Team Leader, Care Team, and Interdisciplinary Team, assists with the development and implementation of a plan of care to meet the needs of participants to remain living safely in the community for as long as possible in alignment with their goals of care. Assists in maintains aligned, comprehensive plan of care for all participants living in residential facilities and coordinates care between facility and Care/Interdisciplinary Team.


• Conducts nursing evaluations as needed and coordinates with the Care/Interdisciplinary Team to develop a comprehensive plan of care for each participant.


• Performs general and skilled nursing care to participants as assigned. Implements the plan of care and physician orders in coordination with the Care/Interdisciplinary Team. Observes, records, and reports participant's condition and reaction to drugs and treatments to providers. Triages participants to determine priority of care and assists in the treatment, examination, and testing of participants.


• Participates in nursing on-call rotation to provide after-hours and weekend assistance and care, as needed, to PACE participants.


• Receives participant complaints, triages, and responds to participant needs.


• Reviews and signs off orders and disseminates orders and instructions to appropriate staff and contracted personnel. Maintains and reviews participant records, charts, and other pertinent information. Completes documentation and charting in accordance with organizational policy.


• Working with the Care Team Leader, performs care coordination with outside providers and facilities including hospitals, nursing homes, assisted living facilities, and physician/provider offices. May accompany participants to specialist appointments or make hospital or other facility visits when necessary for coordination of participant care.


• Provides treatment instructions and nursing education to participants and family.


• Assists, as directed, in the maintenance of the clinic and exam rooms including stocking of supplies and materials and cleanliness.


• Is expected to consistently practice in alignment with program cultural/behavioral standards (Fundamentals).


• Acts within the scope of his or her authority to practice


• Follows all Senior CommUnity Care policies and procedures and Occupational Safety and Health Administration (OSHA) safety guidelines.


• Handles potentially infectious specimens with appropriate biohazard precautions, and practices Universal Precautions.


• Maintains safe working environment. Follows Senior CommUnity Care Safety policies and procedures.


• Protects privacy and maintains confidentiality of all company procedures, results and information about employees, participants and families.


• Participates in and supports Quality Improvement Initiatives.


• Participates in continuing education classes and any required staff and training meetings. Maintains professional affiliations and any required certifications.


• Performs other duties as required.


Job Requirements:



Required Qualifications:



• Must have medical clearance for communicable diseases and up-to-date immunizations before having direct participant contact.


• Must have a valid driver's license, proof of insurance and have means of transportation.


• Graduate of a school of professional nursing required.


• Current Colorado Practical Nurses License (L.P.N.), Cardio Pulmonary Resuscitation (CPR) certification required.


• A minimum of one year's experience in working with the frail or elderly population required. Intravenous Venous (IV) Certification preferred.



Preferred Qualifications:



• Intravenous Venous (IV) Certification preferred.



Knowledge, Skills and Abilities

• Broad knowledge base of health care problems of the frail elderly.
• Knowledge of medical equipment and instruments.
• Knowledge of common safety hazards and precautions to establish a safe work environment.
• Skilled in identifying problems and recommending solutions.
• Skilled in preparing and maintaining records and writing reports, and responding to correspondence.
• Skilled in developing and maintaining department quality assurance.
• Skilled in establishing and maintaining effective working relationships with participants, medical staff, staff members and family caregivers.
• Ability to react calmly and effectively in emergency situations.
• Knowledge of medications used in geriatrics and adverse reactions.
• Skilled in wound care.
