Job Description:

 

Scope of position:

Provide clinical leadership and oversight, in collaboration with the psychiatric care provider(s), to the ACT program, and supervise and manage the team operations and staffing. Ensures the systematic provision of treatment and social services such as case management, individual, group and family therapy. Team Leader is responsible for communication among staff, consumers, families, and external resources for collaboration and continuity of treatment; actively participates as a part of a multi-disciplinary treatment team. Responsible for assessment, treatment and discharge planning. Team Leader must present self and service line in a knowledgeable and professional manner. Team Lead maintains positive relationship with referral sources, stakeholders, payors and TMACT.

Principal Results Expected: 
Team Leader is responsible for ensuring that team meets and maintains identified census targets and required ACTT visits per service definition; Team Leader is responsible for ensuring team cohesion, staff retention and that TMACT scores meet agency standards.

Profile: 
Graduate Degree in Human Service or related field from an accredited college or university is required. The team leader shall be a licensed mental health professional holding any of the following licenses: licensed psychologist, licensed psychological associate, licensed clinical social worker, licensed professional counselor, licensed marriage and family therapist, licensed psychiatric nurse practitioner, clinical nurse specialist certified as an advanced practice psychiatric clinical nurse specialist.

Experience:
The team leader shall have three years of clinical experience with severe and persistent mental illness, with a minimum of 2 years post-graduate school. Five years’ clinical experience with severe and persistent mental illness preferred. Experience also to include: family, group, and individual therapy/counseling; must be experienced with the supervision and management of SA/MH services within a variety of modalities and populations.

Must possess initiative and judgment capabilities to organize and plan activities, systematize procedures, promote favorable public relations and make decisions affecting service delivery to consumers. Must be flexible to adjust to changing conditions and the various details of the job.
